Objective: The paper is aimed to get a reference for the clinical formulation of standardized treatment and targeted nursing,by evaluating the effect of the "Opioid Induced Constipation Assessment Scale Among Cancer Patients",investigating the incidence of severity of Opioid Induced Constipation(OIC)in cancer patients,and exploring its correlation with Karnofsky,Performance Status,opioid drugs,psychological and physical pain level,and analyzing the factors of affecting the rate of opioid induced constipation severity in cancer patients.Method: This study applied a random sampling method.Patients with opioid induced constipation from a tertiary hospital in Liaoning Province were randomly selected to complete a questionnaire survey from October 2017 to October 2018.The research tools included a general information questionnaire designed by the research team,a Opioid Induced Constipation Assessment Scale in Cancer Patients,a Numberical Rating Scale(NRS),Distress Thermometer(DT),Karnofsky(KPS),and Performance Status(PS).Excel was used to establish the database,and SPSS23.0 software for analysis.Specific methods: Counting data were expressed by examples and constituent ratios,and measurement data were indicated by mean ± standard deviation.Nonparametric rank sum test and variance showed the univariate analysis.Pearson and Spearman correlation analysis presented correlation analysis.And ordered logistic regression analysis performed the multivariate analysis.Result: 1 "Opioid Induced Constipation Assessment Scale in Cancer Patients " can accurately reflect the level of OIC in cancer patients,and effectively distinguish the severity of OIC among patients(P< 0.001).2 The severity of OIC in cancer patients 1)29 cases(15.0%)with mild constipation,116 cases(60.1%)with moderate constipation,and 48 cases(24.9%)with severe constipation.2)The majority of men are with mild to moderate constipation.On the contrary,more women feel severe constipated.In a conclusion,female patient suffers further from the constipation than the male.3 Univariate analysis of the severity of OIC in cancer patients: there are 9 factors,for instance,body weight,body mass index,daily diet,water intake,exercise,nausea,sleep,fatigue,defecation environment,etc.contributed to the differences which are statistically significant(P<0.05).4 The analysis of correlation factors: The severity of OIC in cancer patients was positively correlated with Oxy Contin,morphine hydrochloride,taylorine dose,and days that patients take opioid orally(r=0.243~0.480,P<0.001),the total score of "Opioid Induced Constipation Assessment Scale in Cancer Patients" and the drug-affected dimension score were negative correlation with Karnofsky score(r=0.148 ~ 0.150,P < 0.05),the total score of the "Opioid Induced Constipation Assessment Scale in Cancer Patients" and the dimensions of the scale were positively correlated with the degree of pain(r=0.326~0.509,P<0.001),the total scores of the "Opioid Induced Constipation Assessment Scale in Cancer Patients" and the dimensions of the scale were positively correlated with the degree of psychological distress(r = 0.411~0.472,P<0.001).5 The ordered logistic regression analysis results of OIC severity of cancer patients showed that the variables of oral days of opioids,Oxy Contin dose,pain level and daily drinking water in the past week entered the regression equation(b=0.005,b =0.008,b =0.476,b=0.633,P<0.05).Conclusion1 "Opioid Induced Constipation Assessment Scale in Cancer Patients" is simple,convenient,clinically practical,compliant,highly operational,and regional culture.And the scale can effectively divide the severity of OIC,which is suitable for clinical application.2 The severity of OIC in cancer patients is mainly concentrated at moderate levels,and the levels of severe and mild constipation are basically the same;the incidence of OIC in cancer patients is higher in men than women,while the incidence of sever OIC is higher in women than men.Therefore,focusing on moderate constipation can effectively delay the progress of constipation severity and reduce the incidence of complications.3 The severity of OIC in cancer patients is related to the total score of KPS,the degree of pain and the degree of psychological distress.That is,the lower quality of life the patient has,the higher degree of pain he suffers,and high degree of psychological distress can increase the severity of OIC.4 The severity of OIC in cancer patients is related to the dose of Oxy Contin,morphine hydrochloride,taylorin,and opioid oral days.The initial dose of opioids is large,and as the days of oral administration longer,the severity of constipation will be increased.5 The large initial oral dose of opioids,long oral days,and severe pain can increase the severity of OIC in cancer patients as well as low water intake.
