Clinical Study On The Comprehensive Treatment Of Extremity Recurrent Liposarcoma

?Objective? Liposarcoma is a rare soft tissue sarcoma derived from mesenchymal tissue,with a high incidence of recurrence and metastasis.Through the retrospective study data of the extremity recurrent liposarcoma with comprehensive treatment,to analysis its clinical efficacy.And explore the factors affecting re-recurrence of extremity soft tissue sarcoma.?Methods? From 1st January,2010 to 31 st December,2013,71 patients(33 cases of myxoid liposarcoma,46.5%;16 cases of well differentiation liposarcoma/ atypical lipomatous tumor,22.5%;17 cases of pleomorphic liposarcoma,23.9%;5 cases of dedifferentiated liposarcoma,7.1%)treated with a single institution-the department of bone and soft tissue tumor department,Shandong Cancer Hospital.42 patients(except the 16 cases of well differentiation liposarcoma/ atypical lipomatous tumor)were treated with a combination of extensive resection combined with radiotherapy and chemotherapy,13 patients were treated with extensive resection alone.Analysis and compare the 5 years overall survival(OS),5years disease-free survival(DFS)and risk factors for re-recurrence were analyzed.?Results? All of the 71 patients were followed up completely,44 males(62.0%),27 females(38.0%);mean age 52.4 years old(from 5 years to 90 years,median age 50 years old);mean follow up 69.5 months;42 cases take comprehensive treatment(extensive resection combined radiotherapy and chemotherapy),13 cases take extensive resection alone;the 5 years OS 77.5% of the patients,5-year DFS of comprehensive group and resection alone group 81.0% VS 46.2%(p<0.01);1.3 times the mean re-recurrent times.The patient age?tumor size?tumor histological classification?measure of treatment?recurrent times were the prognostic factors of the 5years DFS;the tumor histological classification and the measure of treatment were the factors influence the recurrent times(p<0.05).?Conclusion? The comprehensive treatment group could increase the DFS obviously,the comprehensive treatment and the treatment measure decrease the times of re-recurrence.
