Objective:To compare the clinical efficacy of laparoscopic radical cystectomy and open radical cystectomy.Method:A retrospective analysis of 60 cases of patients underwent radical cystectomy were conducted between July 2011 and July 2016 in The First People hospital of Jining.Routine examinations,such as ultrasound,CT or MRI,were performed in all patients before surgery,and cystoscopy and pathological biopsy were underwent further.The patients underwent laparoscopic radical cystectomy or open radical cystectomy after definite diagnosis of bladder cancer.Cases were selected according to the inclusion criteria,and all patients had no distant metastasis,heart,lung,kidney,brain,liver and other important organ diseases.According to different surgical methods,the patients were divided into the laparoscopic group(30 cases)and the open group(30 cases).The operative time,intraoperative blood loss,blood transfusion rate,postoperative recovery,complications and postoperative histopathology of the two surgical methods were statistically analyzed.Results:There was no significant difference in preoperative data between the two groups(P > 0.05),indicating that the two groups had good balance and comparability.The operative time of laparoscopic group and open group were(268.03±55.23)min and(232.63±44.43)min,the intraoperative blood loss were(476.67±478.82)ml and(716.67±417.57)ml,the postoperative anal exhaust time were(2.93±0.98)d and(3.57±0.82)d,the removal time of pelvic drainage tube were(6.23±1.81)d and(7.17±1.29)d,and the postoperative hospitalization time were(11.80±3.70)d and(14.00±3.24)d,the incidence of postoperative complications was 16.67% and 46.67%,and all the above differences were statistically significant(P<0.05).The intraoperative and postoperative blood transfusion rates were 16.67% vs 36.67% and 26.67% vs 40.00%,but there were no statistically significant differences between the two groups in blood transfusion rate,histological type,pathological staging and grading,lymph node metastasis and positive surgical margin(P>0.05).Conclusion:In addition to long operation time,laparoscopic radical cystectomy has the advantages of less surgical trauma,less intraoperative blood loss,faster postoperative recovery,shorter hospital stay and fewer postoperative complications,etc.Besides,the postoperative radical tumor treatment effect and the short-term efficacy is satisfactory.
