Study On Chinese Medicine Constitution,Clinical Pathology And Related Indexes In Patients With Idiopathic Membranous Nephropathy In Guangxi

Objective:To investigate the distribution of pathological staging and laboratory parameters of primary membranous nephropathy in Guangxi,and to summarize the different stages of membranous nephropathy,serum creatinine,24-hour urine protein quantitation,plasma albumin and blood lipids in Guangxi.Methods:Informed consent was obtained from 140 patients with membranous nephropathy who had not been treated with hormones and immunosuppressive agents who were admitted to the hospital from January 2017 to January 2019.Based on the method for judging the nine constitutions in the Classification and Judgment of TCM Constitution,the patient fills in the “Traditional TCM Classification and Judgment Self-Test Form” and calculates the score according to the requirements to determine the patient's constitution type.The patient's renal biopsy results and the recent 24-hour urine protein quantitation,blood lipids,albumin and serum creatinine results were collected and statistical analysis was performed accordingly.Results:(1)There was no significant difference in gender and age of onset in primary membranous nephropathy(P>0.05).The average age of onset of men was 46.84 years old,and the average age of onset of women was 47.58 years old.The minimum age of onset was 18 years old,the maximum was 72 years old,and the number of onset ages was 40-59 years old,accounting for 79 cases,accounting for 56.4% of the total.(2)In primary membranous nephropathy,there was no significant difference in the distribution of gender to TCM constitution(P>0.05).(3)Among the patients with primary membranous nephropathy,the physique distribution was mainly in qi deficiency,yang deficiency,yin deficiency and damp heat,qi deficiency and yang deficiency,accounting for 30.8% and 25.7% respectively;They are yin deficiency,damp heat,blood sputum and phlegm dampness,accounting for 14.3%,10.7%,9.3% and 5.0%,respectively;the least are flat,stagnation and sputum,all less than the total 5 %,accounting for 2.1%,1.4%,0.7%.(4)The pathological stage of patients with primary membranous nephropathy was mainly stage I-II and stage II,of which stage II was the most,reaching 47.1%;the pathological stage of patients with qi deficiency was mainly found in stage I,stage I-II,stage II.They were 20.9%,27.9%,and 44.2%,respectively.The pathological stages of patients with yang deficiency were mainly found in stage I-II and stage II,which were 22.2% and 55.6%,respectively.The pathological stage of patients with yin deficiency was mainly found in stage II and stage II-III.,45.0%,25.0%;wet heat and blood stasis are mainly found in phase II,60.0%,46.1%.There were significant differences in the pathological stage of primary membranous nephropathy between different TCM(P<0.05).(5)There was no significant difference in IMM cholesterol,total triglyceride and low density lipoprotein between different TCM constitutions(P>0.05).(6)The serum creatinine level of patients with yang deficiency is 78.0umol/L(56~203umol/L),P<0.05;the urinary protein content of patients with yang deficiency and dampness primary membranous nephropathy is more than 24 hours.They were 5.95±1.57 g and 5.16±2.06 g,respectively,P<0.05.There were significant differences in serum creatinine,24 h urine protein and plasma albumin in different TCM constitutions(P<0.05).Conclusion:1.The majority of IMN patients in Guangxi are mainly middle-aged and elderly.2.The physical and mental qualities of IMN patients in Guangxi are mainly qi deficiency and yang deficiency.The pathogenesis of qi deficiency is: qi deficiency > yang deficiency > yin deficiency > damp heat > blood sputum > phlegm dampness > peace > qi Quality>Special enamel.3.The pathological stage of IMN patients in Guangxi is mainly stage I-II and stage II,of which stage II is the most;the pathological stage of patients with qi deficiency is mainly seen in stage I,stage I-II and stage II;the pathological stage of patients with yang deficiency is mainly seen in Stages I-II and II;pathological staging of patients with yin deficiency is mainly seen in stage II and stage II-III;moist heat and blood sputum are mainly found in stage II.4.IMN patients with renal function damage may be related to yang deficiency.5.the cause of urinary protein formation in IMN patients may be related to yang deficiency and wet heat constitution.
