Objectives:Understand the etiology and risk factors of accidental injury in emergency patients in Handan area,and provide theoretical basis for formulating effective prevention strategies of regional accidental injury.Methods:All outpatient cases in the emergency department were retrospectively collected in the Affiliated Hospital of Hebei University of Engineering in 2016.The number of all eligible patients were counted,gathered into the Excel software and built into database,including the data such as etiological factors of accidental injury,month,season,clinic time,gender,age,injured area and prognosis.Then SPSS 24.0 software was used to conduct statistical analysis on the above indicators of the selected cases.The main calculation indicators was the composition ratio.The count data were tested by chi-square test and it was statistically significant when P<0.05.The measurement data conforming to the condition of normality and homogeneity of variance adopted (?)±S.In addition,median and interquartile range are used for non-normal distribution.Logistic regression analysis was used to determine the risk factors of accidental injury of different etiologies.Results:A total of 12,830 patients were admitted to the emergency department of the Affiliated Hospital of Hebei University of Engineering in 2016,of which a total of1,943 patients met the filtering conditions above for accidental injury.(1)The ratio of male to female patients in accidental injury was 1.5:1.The mean age of emergency patients was(34±31)years old,and the proportion of emergency patients in accidental injury in spring was 27.54%.The average monthly number of patients was(161.92±21.33),and the peak hours were from 18:00 to 23:59.More patients were susceptible to traffic accident injury and falling injury accounting for 73.03%.(2)Among the injuries of traffic accident injury,falling injury and sharp instrument injury,males were significantly more than females(x~2=48.756,P<0.001).Predilection age was 21-40 years old.Traffic accident injury was common in autumn,falling injury was common in spring and winter,and gas poisoning was common in winter.Traffic accident injury and falling injury were common in all months of the year,and sharp instrument injury was common from May to August.Falling injury was more common in 0:00-2:59,18:00-19:59 and 21:00-23:59,while traffic accident injury was common in the rest of the time.The proportion of various types of accidental injury was statistically significant via chi-square test in different age groups(x~2=474.520,P<0.001),different seasons(x~2=285.059,P<0.001),different months(x~2=578.622,P<0.001)and different time groups(x~2=1009.773,P<0.001).(3)The proportion of various types of accidental injury in different injured area(x~2=869.709,P<0.001)and in different prognosis(x~2=588.713,P<0.001)were statistically significant.The head was the main injured area in the traffic accident and falling injury,and the upper limb was the main injury in the sharp instrument injury.Most of the patients left the hospital except that the patients in drug poisoning,gas poisoning and electrically damaging were be hospitalized.(4)Multivariate logistic regression analysis showed that male were more likely than female to suffer falling injury and sharp instrument injury,increasing the risk of 0.797(95%CI:1.447±2.233)times and 0.728 times(95%CI:1.182±2.526).Conclusions:The accidental injuries were mainly traffic accident injury and falling injury,followed by sharp instrument injury,gas poisoning and other types of accidental injuries.Drug poisoning,food poisoning,drowning,burning,stinging and bite,electrically damaging and acute mechanical asphyxia are rare.There were more males than females,and the age between 21 years old and 40 years old was the age with high incidence of accidental injury.The accidental injury occurred frequently in spring and 18:00-23:59 at night was the peak of seeking medical treatment.Male were more likely than female to suffer falling injury,sharp instrument injury.Emergency accidental injuries were more common in the head,and the majority of patients were out of hospital.
