| Objective: To observe the effect of the acupuncture method of“shaoshanhuo”on Karnofsky scores,cancer-related fatigue piper fatigue rating scale,TCM clinical symptom scores and blood routine index in CRF patients with spleen and kidney yang deficiency;and to evaluate the clinical efficacy of this method in treating CRF.Methods: Cases were selected according to inclusion criteria.108 patients with CRF were randomly divided into three groups,36 cases in each one,respectively named the“shaoshanhuo”acupuncture treatment group(group A),conventional acupuncture group(group B)and support treatment group(group C).Group C received routine hospitalization support treatment,Group B was treated with conventional acupuncture on the basis of support treatment(for filling method),On the basis of support treatment,group A selected the same acupoints as the conventional acupuncture group and use“shaoshanhuo” acupuncture treatment at ST36 and BL23 points.Treatment was given once a day and 10 days was regarded as a course of treatment,with 2 days interval between 2 courses.After 3 courses of continuous treatment,statistical analysis and efficacy evaluation can be made.Results:(1)There are 108 cases gathered records,18 cases losted,90 cases completed actually in the reserch,there are 30 cases in each group.(2)After the treatment,the total effective rate is 83% in group A,67% in group B and 40% in group C.There were statistically significant differences in the total effective rate between the 3 groups(p<0.05).And there was statistically significant difference between group A and group C(P<0.01).Similarly,there was statistically significant difference between group A and group B(P<0.05).The results showed that“ shaoshanhuo ” acupuncture treatment was the best in relieve the cancer-related fatigue,followed by the conventional acupuncture treatment,while the support treatment was the worst among three groups.(3)In Karnofasky peformance status scores: There were statistically significant differences in KPS scores among the three groups before and after treatment(P<0.05).It shows that all three therapies can improve the KPS score as well as the quality of life of patients.Besides,there was statistically significant difference between group A and group C,similarly,between group A and group B(P<0.01).It’s inferred that there were significant differences in the therapeutic effect among the three treatments.“shaoshanhuo” acupuncture is more effective than conventional acupuncture and support treatment in improving KPS score.(4)In Piper fatigue rating scale: There were significant differences in four dimensions,namely behavior,perception,emotion and cognition,as well as total scores of fatigue before and after treatment in group A(P<0.05).There were significant differences in the three dimensions of behavior,perception,emotion and total fatigue scores before and after treatment in group B(P<0.05),but no significant difference in cognitive dimension(P>0.05).There were significant differences in perception,behavior and total fatigue scores before and after treatment in group C(P<0.05),but no significant difference in emotional and cognitive dimensions(P>0.05).The results showed that the three treatments can alleviate fatigue to some extent,but there were differences in the improvement of four dimensions: “shaoshanhuo”acupuncture can improve all the four dimensions;Conventional acupuncture can improve the three dimensions of behavior,perception and emotion,except the cognitive dimension;Support treatment can improve the two dimensions of perception and behavior,except emotion and cognitive dimension.Intergroup comparison: There were significant differences in total fatigue score and four dimensions between group A and group C(P< 0.05).Compared with group B,group A had statistical difference in total fatigue score,behavior and perception dimension (P<0.05),while there was no statistical difference in emotional and cognitive dimension.It can be inferred that there were differences in the effects of the three treatments on fatigue remission.Compared with support treatment,“shaoshanhuo” acupuncture had therapeutic advantages on both emotional and cognitive dimensions,as well as total fatigue scores,behavior and perception dimensions.Compared with conventional acupuncture treatment,“shaoshanhuo” acupuncture had more advantages in improving behavior and perception dimensions.(5)TCM clinical symptom scores: The comparison of the three groups showed that there was significant difference in the scores between group A and group B before and after treatment(P<0.05);but no significant difference in group C(P>0.05).Intergroup comparison: After treatment,the scores of TCM clinical symptoms in three groups were significantly different(P<0.05).Compared with group C and group B,the difference was statistically significant(P<0.01),which showed that both the “shaoshanhuo” acupuncture and the conventional acupuncture are effective in improving the clinical symptoms of spleen and kidney yang deficiency,but the “shaoshanhuo” acupuncture has better curative effect,while the support treatment has no obvious curative effect.(6)Blood routine index: There were significant differences in four blood routine indexes(leucocyte,lymphocyte,hemoglobin and platelet)among the three groups before and after treatment.Intergroup comparison: There were significant differences in leucocyte,lymphocyte and hemoglobin(P<0.05),but there was no significant difference in platelet(P>0.05).Additionally,there were significant differences in leukocyte,hemoglobin and lymphocyte between group A and group C(P<0.05),but no significant difference in platelet(P>0.05).By contrast with group B,there were significant differences in leukocyte and lymphocyte(P<0.05),without significant differences in hemoglobin and platelet(P<0.05).The experimental evidence suggests that the three treatments could improve the four blood routine indexes.However,the three treatments have different effects on improving leukocyte,lymphocyte and platelet,but there is no significant difference in improving platelet(that is,the three treatments have little effect on platelet).Based on theses results we can see that the method of “shaoshanhuo” acupuncture had more advantages than conventional acupuncture in promoting leucocyte and lymphocyte,and had better therapeutic effect than support needling in improving white blood cells,hemoglobin and lymphocyte.Conclusion: Overall,these findings above strongly implicate that“Shaoshanhuo”acupuncture method is effective in treating cancer-related fatigue of spleen and kidney yang deficiency type.It can significantly reduce the level of fatigue,significantly alleviate the clinical symptoms of TCM,hopefully improve the quality of life of patients,as well as improve blood routine indicators.Compared with conventional acupuncture,it has more advantages in improving emotional and cognitive dimensions,improving leukocyte and lymphocyte,and alleviating the clinical symptoms of spleen and kidney yang deficiency in traditional Chinese medicine.It can alleviate the fatigue symptoms of patients in an all-round way.There were no obvious toxic and side effects in the course of treatment,which preliminarily indicated that the method of “ shaoshanhuo ” acupuncture was feasible and worthy of further popularization and application. |
