Objective: Using Ganyong Decoction combined with ultrasound guided puncture drainage for the treatment of patients with bacterial liver abscess,promoting the drainage of abscess after liquefaction,shortening the patient's course of disease,speeding up their recovery,reducing hospital stay and reducing the economic burden of patients.Provide more clinical reference for the treatment of bacterial liver abscess with Chinese and Western medicine.Method: A retrospective analysis from Jan,2013 to Dec,2018 was done at our institution and a total of 95 patients with bacterial liver abscess,who hepatobiliary ultrasonography shows abscesses are not completely liquefied.First,95 patients were divided into two groups according to different treatment plans.Group A was a simple Western medicine treatment with 47 patients.Group B was treated with Ganyong Decoction on the basis of group A treatment.A total of 48 patients were treated.Chinese medicine selection of Ganyong Decoction,which changed from Tounong Decoction,a chinese medicine.Give the patient twice a day orally.Compare the antipyretic time of the two groups of patients,the time it takes for white blood cells to return to normal,Compare the time required for septic liquefaction in both groups Compare the extent of abscess reduction and puncture time between the two groups(the time required for ultrasound guided puncture after hospitalization).Compare the effectiveness and cure rate of the two groups of patients,Compare hospital stays,treatment costs and extubation time(the time from puncture to extubation)between the two groups.Result: 1.Antipyretic time: The average time of fever in group B Ganyong Decoction combined with Western medicine treatment group was(4.8±1.3)d,which was significantly shorter than the average fever time of group A in the western medicine group(6.3±1.1)d.The difference between the data was statistically significant(P<0.05).2.The time required for leukocytes to return to normal: The average time required for leukocytes to return to normal in group B Ganyong Decoction combined with Western medicine was(5.3 ± 1.5)d,which was significantly shorter than the time required for leukocytes to return to normal in group A alone.(7.4±1.7)d,the difference between the two groups was statistically significant(P<0.05).3.Time required for liquefaction of abscess: The average time required for liquefaction of abscess in group B Ganyong Decoction combined with Western medicine treatment group was(4.1±1.5)days,which was significantly shorter than that required in group A alone group(5.7±1.1).The difference between the two groups was statistically significant(P<0.05).4.The extent of hepatic abscess reduction: The average extent of liver abscess was(11.0±2.1)cm when patients in group B were on admission.The average range of liver abscess was about(11.4±1.8)cm when patients in group A were admitted to hospital.There was no significant difference between the two groups(P>0.05).After treatment,the average extent of hepatic abscess in group B was about(6.2±1.1)cm,which was significantly better than that of group A in the western medicine group(3.3±0.8)cm.The difference between the two groups was statistically significant(P< 0.05).5.Puncture time: The average puncture time of group B Ganyong Decoction combined with western medicine treatment group was(5.0±0.8)d,which was significantly shorter than that of group A(7.1±1.1)d.The difference between the two groups was Statistical significance(P <0.05).6.Comparison of curative effect: The effective rate was 95.8% in the 48 patients with group B Ganyong Decoction combined with the western medicine treatment group,and the cure rate was 85.4%.The effective rate of 47 patients in group A was only 91.4%,and the cure rate was 61.7%.There was no significant difference in the efficiency between the two groups(P>0.05).The cure rate in group B was significantly better than that in group A(P<0.05).7.Hospitalization time: The average length of hospital stay in group B Ganyong Decoction combined with Western medicine was(16.3±1.5)days,which was significantly shorter than that in group A(20.1±1.1)days.The difference between the two groups was statistically significant.(P<0.05).8.Treatment costs: The average treatment cost of group B Ganyong Decoction combined with western medicine treatment group was(13573.2±1322.5),which Significantly less than group A simple Western medicine treatment group(17586.5 ± 1221.6).The difference between the two groups was statistically significant.(P<0.05).9.Extubation time: The mean time of extubation in group B Ganyong Decoction combined with western medicine was(16.2±1.5)d,which was significantly shorter than that in group A(21.5±1.1)d.There were statistical differences between the two groups.Academic significance(P<0.05).Conclusion: 1.Ganyong Decoction combined with Western medicine treatment group was significantly better than the Western medicine treatment group in terms of antipyretic time,abscess liquefaction time,abscess size reduction,puncture time,clinical efficacy,hospitalization time and treatment cost.2.Ganyong Decoction combined with simple western medicine treatment group fully utilized the principle of dialectical treatment of traditional Chinese medicine,combined with the basic treatment of western medicine,to give play to the common advantages of traditional Chinese medicine and Western medicine,it is worthy of promotion in clinical work.
