Objective: To analyze the pathogenic bacteria and drug resistance of silicosis complicated with lower respiratory tract infection and guide clinical medication.Methods:A total of 1,500 patients with silicosis complicated and lower respiratory tract infection admitted to our hospital from January 2015 to June 2018 were selected.The results of jobs,dust exposure time,sputum culture and drug sensitivity were collected and analyzed.Results:A total of 319 pathogenic bacteria were isolated from 4 832 sputum specimens of silicosis patients with lower respiratory tract infection.Among them,306 gram-negative bacilli were klebsiella pneumoniae,pseudomonas aeruginosa,acinetobacter baumannii,escherichia coli,etc.All the 55 strains of fungi are candida.And 30 strains of gram-positive cocci were staphylococcus aureus,staphylococcus epidermidis and staphylococcus haemolyticus,etc.There was no statistically significant difference in gram-negative bacilli infection among silicosis patients of different occupational types.There were statistically significant differences in the number of gram-negative bacilli in silicosis patients with different dust exposure time.The drug resistance rate of the three staphylococci to quinuptin,linezolid,vancomycin,tetracycline,tetracycline,tegacycline and furantoin was 0,moxifloxacin and rifampicin was lower than 25%,and penicillin,erythromycin and cleomycin was higher than 57.14%.The drug resistance rate of staphylococcus aureus to penicillin,erythromycin and chloromycin was higher than 66.67%.The drug resistance rate of staphylococcus epidermidis to cefoxitin,penicillin,benzocillin,ciprofloxacin,clonomycin and compound xinnomin was higher than 57.14%.The resistance rate of hemolytic staphylococcus to cefoxitin,penicillin,benzocillin,gentamicin,ciprofloxacin,levofloxacin and erythromycin was up to 100%.The drug resistance rate of klebsiella pneumoniae to ampicillin and piperacillin was higher than 54.97%,but that of other experimental drugs was lower than 21.85%.The drug resistance rate of pseudomonas aeruginosa to ampicillin and its enzyme preparation,cefazolin,cefuroxime,ceftitan,ceftriaxone,furantoin and compound xinnomin were all higher than 71.43% but lower than 14.29% for other experimental drugs.The drug resistance rate of acinetobacter baumannii to ampicillin,piperacillin,tazobactam,cefazoline,cefuroxime,ceftitan,ceftriaxone and furantoin was higher than 56.41%,but lower than 41.03% for other experimental drugs.The drug resistance rate of escherichia coli to ampicillin,ampicillin sulbactam,piperacillin,cefazoline,cefuroxime sodium,ceftriaxone,amitracillin and furantoin was higher than 52.32%,but other experimental drugs was lower than 47.62%.The drug resistance rate of silicosis complicated with lower respiratory tract infection was significantly higher than that of common strains.The drug resistance rate of candida was lower than 33.33%.Conclusion:The most common pathogen of silicosis complicated with lower respiratory tract infection is gram-negative bacillus.There was no difference in gram-negative bacilli infection among different types of silicosis patients.The longer the dust exposure time in silicosis patients,the higher infection rate of the gram-negative bacillus.The possibility of multiple drug-resistant bacterial infections or fungal infections should be considered when the treatment effect is poor.
