Risk Factors For Recurrence After Complex Anal Fistula Surgery

Objective: To summarize the risk factors of recurrence of complex anal fistula after operation,and analyze the different effects of each risk factor on recurrence of complex anal fistula after operation,so as to improve the cure rate of complex anal fistula in our hospital cure rate of operation,and provide theoretical basis for further study of complex anal fistula in our hospital.Methods: A retrospective analysis was made on 358 patients who were admitted to Anorectal Department of the First Affiliated Hospital of Xinjiang Medical University from January 2012 to January 2017.They were diagnosed as complicated anal fistula and treated surgically.The patients were grouped according to different general conditions and conditions.The recurrence and prognosis of the patients were followed up for one year.The related factors of each group were compared and analyzed statistically.Risk factors and independent risk factors affecting recurrence of complex anal fistula after operation were identified.Result: During the follow-up period of one year,296 patients were followed up,51 of whom had recurrence of anal fistula.The recurrence rate was 17.23%.The analysis of influencing factors of recurrence of complex anal fistula showed that the history of operation(OR=20.345,6.668-62.070),the history of thread-drawing(OR=7.679,1.416-41.659),the history of high anal fistula(OR=3.548,1.299-9.696),the left internal orifice(OR=10.106,1.458-70.051)and the right internal orifice(OR=6.489,1.050-40.111)were independent influencing factors of recurrence of complex anal fistula.Conclusion: High complex anal fistula patients,patients with history of anal fistula surgery,patients with history of anal fistula thread hanging,easily lead to high recurrence rate of complex anal fistula after operation.It is concluded that in the diagnosis and treatment of complex anal fistula patients,high recurrence rate patients should be identified,adverse effects of iatrogenic factors should be avoided as far as possible,preoperative examination and preoperative preparation of complex anal fistula should be improved,scientific and reasonable surgical treatment methods should beselected,location and number of internal orifices of anal fistula should be defined,rehabilitation nursing and dressing change should be strengthened,and complexity should be informed.Good healing of anal fistula after operation and related matters needing attention for cure are of great significance to further reduce the recurrence of complex anal fistula after operation and to improve the one-time cure rate of complex anal fistula in clinic.
