Objective:To study the expression level of CD3~+CD38~+T lymphocyte in peripheral blood,the changes of biochemical indexes and the correlation between CD3~+CD38~+T lymphocyte expression and viral load in HIV/HBV co-infected population.Methods:147 patients were divided into HIV group(49 cases),HBV group(50 cases)and HIV/HBV group(48 cases).CD3~+,CD3~+CD4~+,CD3~+CD8~+,CD3~+CD38~+were detected by flow cytometry.The results of liver function,kidney function,HIV and HBV viral load were collected and analyzed.The expression level of CD38 in HAART and anti-HBV stages and the correlation with viral load were obtained.SPSS 19.0 statistical software was used to process data.Results:In HBV group,there was no significant difference in CD3~+?CD3~+CD4~+T lymphocyte count and ALB,GLO,BUN,Cr,WBC,HGB,PLT in different antiviral stages(P>0.05);CD3~+CD8~+?CD3~+CD38~+T lymphocyte count,HBV DNA level,ALT,AST,TBil,DBil in different stages of anti-virus were significantly different(P<0.01),CD3~+CD8~+T lymphocyte count and HBV DNA decreased with the prolongation of antiviral time.In HIV group,there were significant differences in,HIV RNA level of CD3~+?CD3~+CD4~+?CD3~+CD8~+?CD3~+CD38~+T lymphocyte count and AST,ALB,TBil,DBil at different stages of HAART(P<0.01),The number of CD3~+CD38~+T lymphocytes and HIV RNA,AST decreased with the prolongation of HAART time,CD3~+CD4~+T lymphocyte count increased continuously,ALB increased most obviously at 12 months,There was no significant difference in the other indexes between different antiviral stages(P>0.05).In HIV/HBV group,CD3~+?CD3~+CD4~+?CD3~+CD8~+?CD3~+CD38~+T lymphocyte count and HIV RNA,HBV DNA(log),DBil were significantly different in HAART and anti-HBV at different time(P<0.01),The number of CD3~+CD4~+T lymphocytes increased with the prolongation of HAART and anti HBV,and the counts of CD3~+CD8~+,CD3~+CD38~+T lymphocytes and HIV RNA,HBV DNA(log),DBil decreased with the prolongation of HAART and anti HBV time,There was no significant difference in the other indexes between different antiviral stages(P>0.05).Different antiviral time in HIV group and HIV/HBV group:Before antiviral,the difference of CD3~+T lymphocyte count was statistically significant(P<0.05),CD3~+CD4~+T lymphocyte count,ALB,TBil,DBil difference had significant statistical significance(P<0.01),the other indicators were not statistically significant(P>0.05);At 3 months of antiviral therapy,CD3~+CD4~+T lymphocyte count,ALT,TBil had significant statistical significance(P<0.01),ALB had significant difference(P<0.05),and other indicators had no statistical significance(P>0.05).At 6 months of antiviral therapy,there were significant differences in HBV DNA(log)level(P<0.05),CD3~+?CD3~+CD4~+?CD3~+CD8~+?CD3~+CD38~+T lymphocyte count and HIV RNA(log),ALT and ALB levels(P<0.01),while there were no significant differences in other indicators(P>0.05).At 12 months of antiviral therapy,CD3~+?CD3~+CD4~+?CD3~+CD8~+?CD3~+CD38~+T lymphocyte count and HIV RNA(log)level,ALT,ALB,TBil and DBil had significant statistical significance(P<0.01),while the other indicators had no statistical significance(P>0.05).HIV group:CD3~+CD38~+T lymphocyte count was positively correlated with HIV RNA(r=0.721,P<0.01);HBV group:CD3~+CD38~+T lymphocyte count was positively correlated with HBV DNA(r=0.226,P<0.01);HIV/HBV group:CD3~+CD38~+T lymphocyte count was positively correlated with HIV RNA(r=0.700,P<0.01),and was positively correlated with HBV DNA(r=0.637,P<0.01).Conclusions:(1)The expression levels of CD3~+CD8~+,CD3~+CD38~+T lymphocyte are related to the progress of HIV infection,and it tends to increase with the progress of the disease,which can be used as predictors of disease prognosis;(2)The expression level of CD3~+CD4~+T lymphocyte in patients with HIV/HBV co-infection is lower than that in patients with HIV simple infection.The former has more severe liver injury,slower recovery of liver function and slower clearance of HIV and HBV;(3)In HIV/HBV co-infected patients,CD3~+CD38~+T lymphocyte counts have a reference role in assessing HIV and HBV viral load.
