Objective: To explore the efficacy and efficacy of integrated traditional Chinese and Western medicine in the treatment of advanced primary liver cancer, and observe the clinical efficacy of Jianpi Xiao Ji Fang in the treatment of advanced primary liver cancer,so as to provide theoretical basis for the treatment of advanced primary liver cancer. Two methods: 1.comprehensive retrieval system of traditional Chinese medicine and Western medicine in the treatment of advanced hepatocellular carcinoma: a randomized controlled clinical study, the use of NoteExpress for the preliminary document re scheduling, download the literatures met the inclusion criteria for full text reading after further re scheduling, will be fully integrated into the literature data extraction, data were analyzed by Revman 5.3 software.Through the observation of 2.between 2016 and 2017 in the Affiliated Hospital of Shandong University of Traditional Chinese Medicine in 60 cases of patients with advanced primary liver cancer, using non randomized open study patients were divided into experimental group of 30 patients given Jianpi Xiaoji decoction combined with conventional western medicine treatment;30 cases in control group were given western medicine conventional treatment.According to the short-term curative effect of the tumor after treatment, the effective rate and stability rate, the quality of life improvement rate and laboratory index according to the short-term curative effect evaluation criteria of WHO,the quality of life improvement and laboratory indicators were statistically evaluated in the two groups. Results: A total of 39 randomized controlled literatures were included in the 1.system reviews. There are 37 articles reported on the main outcome measures,18 of which reported the improvement of quality of life in advanced primary liver cancer,10 reported survival rate, and 9 reported adverse reactions. There were 39 articles on the secondary outcome measures,of which 19 reported short-term efficacy,11 reported alpha fetoprotein, and 9 reported changes in liver function after treatment.Quality of life: 13 literatures analysis of quality of life before and after treatment the mean + standard deviation, due to the heterogeneity test results is too high so take the descriptive analysis, combining traditional Chinese and Western medicine treatment is better than western medicine treatment results;analysis of the quality of life of clinical improvement rate of 5 articles, the heterogeneity test has good homogeneity, the combination of traditional Chinese and Western medicine clinical improvement rate is better than pure western medicine in the treatment of [RR(95%Cl):1.42(1.25,1.61),P<0.00001].The survival rate: 6 reported patients survival rate of 6 months;8 reported the 1 year survival rate of patients;4 reported the 2 year survival rate, respectively. The heterogeneity test showed good homogeneity, the results showed: in the 6 months,1 years and two years survival rate was significantly better than that of simple western medicine treatment the combination of TCM and Western medicine.[RR(95%Cl): 1.13(1.03,1.24),P=0.008];[RR(95%Cl): 1.33(1.18,1.49),P<0.00001];[RR(95%Cl): 1.70(1.33,2.18)].Adverse reactions: in the literature,a total of 9 literatures mention adverse reaction, through the analysis of combined treatment of TCM and Western medicine and simple western medicine treatment compared, results showed that: in the fever, bone marrow suppression, liver function damage, gastrointestinal reactions, treatment significantly lower than that of control group with combination of TCM and Western medicine.[Tau2=0.04,RR(95%Cl): 0.61(0.48,0.78),P<0.00001];[RR(95%Cl): 0.66(0.53,0.83),P<0.00001];[RR(95%Cl): 0.68(P<0.00001])].Recent curative effect: 19 literature related to the short-term efficacy of the tumor body efficiency. The heterogeneity test has good homogeneity. Results the effective rate of integrated traditional Chinese and Western medicine was better than that of Western Medicine([RR(95%Cl)): 1.42(1.30,1.55)P<0.00001].AFP: 8 statistics before and after the treatment of AFP mean standard deviation, the test for heterogeneity exists great heterogeneity,so give up the merger, descriptive analysis, statistical analysis results, after the treatment of combined traditional Chinese and Western medicine group of AFP decreased significantly better than the control group. The serum biochemical indexes before and after treatment: 9 literatures statistics ALT mean standard deviation;7 articles before and after the treatment of AST statistical mean standard deviation;5 articles before and after the treatment of TBil statistical mean standard deviation;4 articles before and after the treatment of ALB statistical mean standard deviation, respectively for heterogeneity test, there is heterogeneity great,so give up with descriptive analysis. Conclusions:The results of the statistical analysis, after the treatment of combining traditional Chinese and Western medicine treatment group in reducing ALT,AST,TBil and ALB were significantly better than the pure western medicine treatment.2.the clinical observation of Jianpi Xiao Ji Fang confirmed that: Based on the inclusion of 60 patients with advanced hepatocellular carcinoma patients before and after imaging, the quality of life and laboratory indexes were statistically analyzed, results in the efficacy rate, stable life quality improvement rate of Jianpi Xiaoji decoction combined with western medicine symptomatic treatment and simple western medicine symptomatic treatment, compared with significant difference between two groups(P<0.05);in the aspect of reducing AFP Jianpi Xiaoji Decoction in the treatment group and control group before and after treatment were not statistically significant(P>0.05).Four conclusion: 1.the combination of traditional Chinese medicine and Western medicine has obvious efficacy, excellent efficacy and safety in the treatment of advanced primary liver cancer.Combination of TCM and Western medicine can improve the quality of life, improve survival rate, alleviate adverse reactions and reduce laboratory indexes. It is worthy of further popularization and application.2.Jianpi Xiao Ji decoction can improve the quality of life and stability of the tumor in patients with middle and late primary liver cancer.
