| Objective:To observe the clinical curative effect of self-made Xiaoxian prescription combined with acupoint application in treating adenoid hypertrophy of phlegm and qi stagnation type in order to provide a feasible basis for the treatment of adenoid hypertrophy with combination of internal and external Chinese medicine.Methods:This research case comes from Shandong province traditional Chinese medicine science and technology development project Self-made Xiaoxian patch local application combined with Chinese medicine oral treatment of children with adenoid hypertrophy(phlegm coagulation qi stagnation)clinical observation.(the project numbert is 2017-111),the subjects of the study were 93 children who met the diagnostic criteria and included in the paediatric outpatient and inpatient treatment in the second affiliated Hospital of Shandong University of traditional Chinese Medicine.Divide all cases into three groups according to the random numerical table method.The combined treatment group was treated with self-made Xiaoxian prescription and acupoint application,the acupoint application group was treated with self-made Xiaoxian prescription acupoint application,while the control group was treated with montelukast.The course of treatment was one month,observe the curative effect indexes such as TCM syndrome score,quality of life score after treatment,and the recurrence rate was observed at the end of treatment for 6 months.Results:1.Among the 93 children,56 were males and 37 were females,30 patients aged from2 to 4.5 years old,41 patients were under the age of seven,22 patients were under the age of nine,28 patients with mild hypertrophy,48 patients with moderate hypertrophy and 17patients with severe hypertrophy.2.Total efficiency:The total effective rate was 67.74%in the acupoint application group,93.75%in the combined treatment group and 70.00%in the control group.The results of the combined group treatment compared respectively with the acupoint application group and the control group were?~2=6.907,P=0.009 and?~2=5.984,P=0.014,P<0.017,the difference has statistically significant.Comparison between acupoint application group and control group,?~2=0.036,P=0.849,P>0.017,the difference was not statistically significant.3.Main symptoms:Comparison of the main syndromes before treatment in the three groups,P>0.017,the difference was not statistically significant.Comparison of nasal congestion,open mouth respiratory and snoring before and after treatment in three groups,P<0.05,the difference has statistically significant.Comparison of tongue image before and after treatment in combined treatment group and acupoint application group,P<0.05,the difference has statistically significant.Comparison of tongue image before and after treatment in control group,P>0.05,the difference was not statistically significant.Pairwise comparison between the three groups after treatment:Results of improving nasal congestion symptom score by contrast in three groups,P>0.017,the difference was not statistically significant.Comparison of symptoms of open mouth respiratory in combined treatment group and control group,P>0.017,the difference was not statistically significant,comparison between two groups and acupoint application group,P<0.017,the difference has statistically significant.Improvement of snoring and tongue image score in the combined treatment group compared with the other two groups,P<0.017,the difference has statistically significant,comparison of symptoms of snoring in acupoint application group and control group,P>0.017,the difference was not statistically significant,compare-son of tongue image symptoms,P<0.017,the difference has statistically significant.4.Secondary symptoms:Comparison of three groups before treatment,P>0.017,the difference was not statistically significant.Comparison of three groups before and after treatment,P<0.05,the difference has statistically significant.Pairwise comparison between the three groups after treatment:Acupoint application group and combined treatment group,Z=-2.591,P=0.010,control group and combined treatment group,Z=-2.518,P=0.012,P<0.017,the difference has statistically significant.Control group and acupoint application group,Z=-0.725,P=0.469,P>0.017,the results were not statistically significant.5.Quality of life survey:Comparison of three groups before treatment,P>0.017,the difference was not statistically significant.Comparison of three groups before and after treatment,P<0.05,the difference has statistically significant.Pairwise comparison between the three groups after treatment:Acupoint application group and combined treatment group,Z=-3.147,P=0.002,control group and combined treatment group,Z=-2.541,P=0.011,P<0.017,the difference has statistically significant.Control group and acupoint application group,Z=-0.217,P=0.828,P>0.017,the difference was not statistically significant.6.Follow up:84 of 93 cases were followed up to 6 months.Combined treatment group 29 cases,recurrence 5 cases.Acupoint application group 28 cases,recurrence 7cases.Control group 27 cases,recurrence 8 cases.Comparison of recurrence rate among three groups,P>0.017,the difference was not statistically significant.Conclusion:1.The total effective rate of the combined treatment group was the highest,and the total effective rate of the acupoint application group and the control group was similar.2.The three groups can improve nasal congestion,open mouth respiratory,snoring.The curative effect of the three groups in improving nasal congestion is similar.The curative effect of the combined treatment group and the control group on the improvement of open mouth respiration was similar and better than that of the acupoint application group.The combined treatment group was superior to the other two groups in improving snoring.Both the combined treatment group and the acupoint application group could improve the tongue image,and the former was better than the latter.In the control group,there was no obvious improvement in tongue image.The above comparison has statistically significant.3.Three groups can improve the secondary symptoms and quality of life,the combined treatment group is better than the other two groups,the difference has statistically significant.4.The long-term control of adenoid hypertrophy was effective in all three groups,but the difference was not statistically significant.in the recurrence rate of the three groups during 6-month follow-up.To sum up,the self-made Xiaoxian prescription combined with acupoint application can obviously improve the symptoms of nasal congestion,open mouth respiratory,snoring and tongue image in children with adenoid hypertrophy,improve the quality of life of the children,increase the cure rate,and have a good long-term effect. |
