Objectives(1)To investigate the self-management ability of patients with coronary heart disease and its influencing factors;(2)To investigate the blood lipid status of patients with coronary heart disease;(3)To explore the correlation between self-management ability and blood lipid status of patients with coronary heart disease,so as to provide scientific basis for strengthening health education and personalized nursing intervention in hospital.Methods:In this study,204 patients with coronary heart disease who met the inclusion criteria in a tertiary A hospital in Changsha City from December 2017 to May 2018 were selected as the subjects by the method of target sampling.General information?disease-related knowledge questionnaire,coronary heart disease self-management behavior scale and blood lipid index were used to investigate.All the data were analyzed by SPSS19.0 software.Pearson correlation was used to analyze the correlation between self-management ability and blood lipid status.Statistical test significance level was expressed as P?0.05Results:(1)The total score of self-management ability of patients with coronary heart disease ranged from 27 to 135,with an average score of(80.21+12.04)and a score index of 57.94%.The medical management dimension of disease was the worst among the three dimensions,with an average score of(41.06+6.25)and a score index of 54.25%.The management dimension of daily life was the best.The score range was 8-40 points,averaging(27.16+4.81)and scoring index 67.83%.(2)Among the total score of self-management ability in patients with coronary heart disease,52.94%were in a low level,while only 7.84%were in a high level.In the dimension of disease medical management,55.39%of patients in low-level state had low proportion of patients in middle-level and high-level.In the dimension of daily life management and emotional management,the proportion of patients in middle-level was higher than that in low-level and high-level patients in the same dimension.(3)The analysis of influencing factors of self-management ability of patients with coronary heart disease found that there were significant differences in total scores of self-management ability among patients with age,education level,family income per capita,course of disease,complications,ways of controlling blood lipid and taking lipid-regulating drugs(P?0.05).The stepwise regression analysis of multiple factors showed that the influence was significant(P?0.05).The main correlative factors of self-management ability of patients with coronary heart disease are age,education level,family income per capita and course of disease.(4)TC(4.75±1.13)mmol/L),TG(1.60±0.62)mmol/L,HDL-C(1.12±0.38)mmol/L,LDL-C(3.11±1.02)mmol/L)were studied in patients with coronary heart disease.The incidence of TC abnormality was 64,31.37%,TG abnormality was 89,43.63%,HDL-C abnormality was 97,47.55%,LDL-C abnormality was 85,41.6%.Among them,a total of 157 cases(76.96%)did not reach the LDL-C target value.(5)There were significant differences in serum TC,TG,HDL-C and LDL-C levels among different self-management ability scores of patients with coronary heart disease(P?0.05).Among them,the serum TC,TG and LDL-C indexes of patients with high self-management ability were lower than those of patients with middle and low self-management ability(P?0.05),while the serum HDL-C indexes were higher than those of patients with middle and low self-management ability(P?0.05).Force patients(P?0.05).TC,TG,HDL-C and LDL-C were used as dependent variables,daily life management,disease medicine management and emotional cognitive management scores in self-management behavior were taken as independent variables,and stepwise regression analysis was carried out by stepwise introduction-elimination method(the criteria for introduction and elimination were 0.05 and 0.10,respectively).The results showed that daily life management,disease medicine management and emotional cognitive management in self-management behavior were all influencing factors of TC,TG,HDL-C and LDL-C.Conclusions:(1)The self-management ability of patients with coronary heart disease is at a low level,and the dimension of disease medical management is the worst,and the dimension of daily life management is the best.The main correlative factors of self-management ability of patients with coronary heart disease are age,education level,family income per capita and course of disease.(2)The score of self-management behavior of patients with coronary heart disease is negatively correlated with TC,TG and LDL-C levels,and positively correlated with HDL-C.Daily life management,disease medicine management and emotional cognitive management are all influencing factors of TC,TG,HDL-C and LDL-C in self-management behavior of patients with coronary heart disease.The abnormal blood lipid status of patients with coronary heart disease can be improved by strengthening self-management behavior of patients.
