Research And Practice On Risk Control Of High-risk Drugs

Researchbackground High-risk drugs,also known as high-alert drugs,refer to drugs that have relatively prominent pharmacological effects,fast onset,and can be threatened by improper use.Recently,the risk of drug use caused by high-risk drugs has received much attention,and many foreign countries have begun to focus on the standardized management of high-risk drugs.China dose not impose mandatory restrictions on high-risk drugs,and it does not have a sound management system and working model.Clinical management is relatively weak.It is not uncommon for a patient to be seriously injured or killed due to improper use.Therefore,reducing the risk of drug use in high-risk drugs and reducing the occurrence of drug errors are urgent problems for hospitals.Objective Establish high-risk drug catalogues by screening high-risk drug varieties.Set up special counters,identification management and many other management solutions.Develop management software related to high-risk drugs and establish a complete high-risk drug management system.Through the above measures,the management of high-risk drugs is used throughout the medical process to ensure the safety and effectiveness of clinical drugs.Methods Referring to the list of high-risk drugs issued by ISMP and the "high-risk drugs classification management strategy and recommended catalogue" issued by the Pharmacy Committee of the Chinese Pharmaceutical Association Hospital,combined with the reality of our hospital,the list of high-risk drugs could be formulated originally.Questionnaire survey and Delphi expert consultation were conducted in the clinical department,and the data was collected.The High-risk Drugs List of Jiangsu Shengze Hospital(hereinafter referred to as "our hospital")was sorted out.On the basis of confirmed high-risk drugs directory,Sqlserver2008r2 database and C#program were used to design the software of high-risk drugs in our hospital.The management sections of high-risk drugs were managed by computer.According to pertinent literature and the specific measures of high-risk drugs management in our hospital,the feasible methods of high-risk drugs administration were analyzed.Take high-risk drug warfarin as an example,on the basis of systematic management,clinical pharmacists participated in anticoagulation management and observed the effect of anticoagulation management.Twenty patients with atrial fibrillation admitted to our department of cardiology were treated with warfarin anticoagulant therapy.They were randomly divided into a guidance group and a control group,each with 10 cases.They were treated with clinical pharmacists' pharmacological monitoring and physician directed anticoagulant therapy.Folow-up surveys were conducted at 0,6,and12 months for 1year.The medication compliance was measured.The patient's international normaized ratio(INR)and the complications occurred were recorded.Results Our hospital has established a catalogue of high-risk drugs,containing 25 types of high-risk drugs,including 115 kinds of western medicines and 22 kinds of traditional Chinese medicine injections.It included 137 kinds of drugs in total.Based on the catalogue,the high-risk drug management system software consisting of basic information management module,information maintenance module,examination module,warning module and statistics module was developed.Conclusion The management and risk prevention of high-risk drugs is a major issue faced by medical institutions.Our hospital adopts appropriate methods and measures to standardize the management of high-risk drugs,ensure patients' drug safety,and minimize the occurrence of drug accidents,so as to achieve the goal of safety management.
