ObjectiveTo investigate the incidence of depression disorders in middle-aged patients with acute myocardial infarction afte PCI,the characteristics and influencing factors were analyzed.To provide theoretical and practical basis for the treatment of middle-aged patients with acute myocardial infarction after stent implantation with depression and mood disorders.Methods1.According to inclusion and exclusion criteria,378 middle-aged patients with acute myocardial infarction undergoing PCI for the first time in Jining First People's Hospital from January 1,2016 to July 31,2017 were selected and recorded in detail The specific status of patients and the results of the examination,all subjects were general information survey,including:age,gender,occupation,education level,health insurance type,monthly income,history of smoking and drinking,history of hypertension,history of diabetes.All patients were record the results of auxiliary examinations.set up a database and conduct statistical analysis on the investigation contents.2.Self-rating depression scale(SDS)was used to evaluate depression in patients with acute myocardial infarction at two weeks after percutaneous coronary intervention.Results1.The incidence of depression in middle-aged patients with acute myocardial infarction after percutaneous coronary stent implantation was 37.57%,mild depression was 23.02%,moderate depression was 11.38%and severe depression was 3.17%.2.Middle-aged patients with acute myocardial infarction complicated by depression appear the following characteristics:the age(41-48 years old,49-55 years old,56-65 years old three depression incidence rates were:53.57%,42.03%,29.35%,(x~2=17.68,P<0.01).The incidence of depression for male is 31.44%and female 46.98%,x~2=5.42,P=0.02.Professional for peasants,workers and cadres of the three kinds of patients,the incidence of depression was46.59%,31.54%,20.59%,x~2=9.10,P=0.01.Middle school and below,middle school or high school and above,the incidence of depression was 51.46%,36.65%,22.22%,x~2=30.68,P<0.01.The incidence rates of depression in rural medical insurance and urban medical insurance respectively were 46.02%and 30.20%,x~2=14.61,P<0.01.Patients with income of less than or equal to 3000 yuan/month,3000~5000 yuan/month,more than 5000 yuan and above,the incidence of depression was 51.41%,35.05%,12.90%,x~2=93.87,P<0.01,and the number of stent placement for 1,2,3 and above,the incidence of depression was 28.22%,42.68%,50%,x~2=16.62,P<0.01.The difference was statistically significant(P<0.01),it can be considered as the rate of depressionis higher in younger patients in women and in peasants.It is also higher in patients with lower education level,with rural health insurance,with low monthly income and with more stents implanted.There are differences in the incidence of depression.3.Previous merger of other diseases:hypertension incidence of depression is36.11%,without hypertension incidence is 38.89%(x~2=0.87,P=0.35);diabetes incidence of depression is 39.11%,without diabetes incidence is 36.18%(x~2=0.05,P=0.83).hyperlipidemia incidence of depression is 42.68%,without hyperlipidemia incidence is 33.64%(x~2=0.21,P=0.65).There was no significant difference in the probability of occurrence of depression(P>0.05).It is suggested that patients with percutaneous coronary artery stenting of acute myocardial infarction may be considered as having normal blood pressure and hypertension,normal blood glucose and diabetes mellitus,normal blood lipids and hyperlipidemia No difference in incidence distribution.Conclusion1.The incidence of depression in middle-aged acute myocardial infarction patients after PCI was 37.57%,mild depression was 23.02%,moderate depression was 11.38%and severe depression was 3.17%.2.Patients with AMI have younger age,women and low education level.The incidence of depression is higher in younger patients in women and in peasants.It is also higher in patients with lower education level,with rural health insurance,with low monthly income and with more stents implanted.These people should be given psychological counseling to reduce the incidence of post-PCI depression.and should be promptly given psychological dredging to reduce the incidence of post-PCI depression.3.The incidence of depression has no difference between patients with hypertension or not,diabetes or not,hyperlipidemia or not,but in clinical should actively treat complications to reduce the incidence of cardiovascular events.
