Clinical Observation Of The Prevention Of Huoxueyangxin Decoction In Preventing Postoperative Delirium Of Elder Intertrochanteric Fracture

Objective: To evaluate the clinical prevention and safety of Huoxueyangxin decoction in preventing postoperative Delirium of elderly intertrochanteric fracture through gathering and analyzing the clinical data before and after surgery of elder intertrochanteric fracture and provide clinical basis of preventing measure effectively.Methods: Selecting120 patients who tally accorded with inclusion criteria and do not accorded with exclusion criteria.60 patients were divided into experimental group based on table of random number and others were also divided into control group based on table of random number.The control group gave the basic western measure including traction on femoral supracondylar,keeping balance of blood oxygen saturation,relieving pain,ensruing the normal sleep cycle before the surgery;Giving the normal antibacterial,giving Low Molecular Heparin,0.3mg every day,intramuscular injection,early functional training.The experimental group had the huoxueyangxintang decoction based on the control group since the first day in hospital.14 days was a treatment cycle.Using CAM(Confusion Assessment Method)established by APA,American Psychiatric Association diagnose Delirium.Using DRS(Delirium Rating Scale)evaluate the degree of severity of Delirium when Delirium occurred.Gathering the blood sample in four point-in-time(the first day in hospital,first day,fifth day,tenth day after the surgery)to text index of hemoglobin.The results were analyzed by statistical software SPSS20.0.Results: Totally,137 patients were included in this experiment and the clinical data were collected,the experimental group collected 69 patients and the control group collected 68 patients.1.Postoperative of Delirium: 10 patients occurred Delirium in experimental group,21 patients occurred Delirium in control group.There was statistical significance between two groups(P=0.022).2.The degree of DRS: The DRS scores of experimental group(25.69±4.54)was lower than that of control group(27.41±3.53),There was statistical significance between two groups(P=0.013)..3.Content of hemoglobin: The difference of content of hemoglobin between two groups had statistical significance before and after operation.There was time effect(P=0.000).There was no statistical significance between two groups of content of hemoglobin in first day(P =0.858).There was statistical significance between two groups of content of hemoglobin had in fifth day(P =0.015).There was no statistical significance between two groups of content of hemoglobin in tenth day(P =0.315).The content of hemoglobin of the experimental group was higher than control group,which meant there was group effect(P =0.038),There was no interaction between time factor and group factor(P =0.350).Conclusion: The method of combining huoxueyangxin decoction with basic western measure between the elderly patients who has intertrochanteric Fracture of the Femur could prevent the Delirium and reduce the degree of severity of Delirium effectively.This method has a more obvious effect on regulating the content of hemoglobin than basic western measure.
