Objective: The aim of this study is to investigate the present status and influenc ing factors of the anxiety disorders of children's guardians in Hubei province, and to provide evidence for the further formulation of intervention to prevent the emergence of anxiety disorder. Methods: This cross-sectional study recruited 3568 vaccinated children's guardians in the primary health care clinics of the study areas during the period from August 2014 to August 2015 by the cluster sampling. Moreover, the assessment tool of anxiety disorders was developed by adding some appropriate items on the basis of the self-rating anxiety scale(SAS).And this study surveys immunized children's immunization information, guardians' demographic information and assessment results of anxiety disorders. At the same time, this study assessed the positive rate of anxiety and its severity degree(mild, moderate and severe)according to the Chinese norms of the general people. And then univariate and multivariate analysis were used to investigate the associated factors and risk factors of the SAS score and anxiety of guardians.Results:(1)The positive rate of anxiety among 3066 guardians is 7.1%(219/3066),besides,the positive rate of the guardians of the unvaccinated children is 15.3%(24/157),but the vaccinated children is 6.7%(195/2909),and the difference was statistically significant(?2=16.546,P<0.01).(2)The univariate analysis indicates that level of education and relationship with children show significant associations with the anxiety of guardians, and similarly finds that educational levels of guardians, the relationship of children and guardians, the birth order of immunized children and occupation of guardians are the associated factors related with the guardians' SAS score. In addition, multivariate analysis of the anxiety status discovers that the odds ratio of the other guardians with appearing anxiety is higher than the parents and grandparents, and the former is more likely to obtain higher SAS score. Moreover, the educational levels are risk factors of the SAS score.(3)In the aspect of immunization situation of children, this study finds out that the vaccination status of childre n is related with age of the guardians, educational background, income, anxiety status, and relationship with children and children's sex.Additional, boy is more likely to be vaccinated than girl, and level of income is the influential factor on children's immunization. Conclusions: Children's guardians(parents, grandparents and other)would showed the anxiety symptom in the process of immunization. Moreover, the low educational level of the guardians is a major risk factor for anxiety in the process of children vaccination, and the other guardians are more likely to appear anxiety compared with the parents. And what's more, the children vaccination coverage of guardians with anxiety seems lower than that of normal guardians.Meanwhile, children's sex and mean month income of guardians are determinants on whether the guardians bring their children to vaccinate or not.As a result, the psychological and demographic factors have an important role in the process of immunization. So timely and effective intervention need to be taken to reduce and even avoid this influence for guaranteeing the successful development of immunization procedure and improving the quality of vaccination.
