Objective: To investigate the association between ApoE(rs7412,rs429358)polymorphisms with glucose,lipid and serum uric acid metabolisms in patients with hypertension and/or coronary heart disease.METHODS: A total of 544 patients with hypertension and/or coronary heart disease were selected from March 2017 to January 2018 in the Department of Cardiovascular Diseases of Fujian Provincial Hospital,and all the patients had never taken or stopped taking lipid-lowering drugs for at least a week.Apo E gene was detected by PCR-fluorescence probe method,then,according to the Apo E genotypes(excluding the E2/E4 genotype),the subjects were divided into three groups(E3/E3 genotype,E2/E2+E2/E3 genotypes,and E3/E4+E4/E4 genotypes)and compared the difference of metabolism between these groups.Results:(1)544 patients were enrolled in the study,including 5 cases(0.92%)with Apo E E2/E2,84 cases(15.44%)with E2/E3,366 cases(67.28%)with Apo E E3/E3,76 cases(13.97%)with Apo E E3/E4,5 cases(0.92%)with Apo E E4/E4,and 8 cases(1.47%)with Apo E E2/E4.The frequencies of E2,E3 and E4 in the overall sample were 9.37%,81.99% and 8.64%,respectively.(2)There were differences in concentrations of total cholesterol(TC),low-density lipoprotein(LDL-C),triglyceride(TG),and high-density lipoprotein(HDL-C)among three groups(P=0.000-0.032).Compared with Apo E-E3 homozygotes,Apo E-E2 carriers had lower TC and LDL-C levels,but higher HDL-C,whereas Apo E-E4 carriers had higher TC,LDL-C,and TG levels.Furthermore,multivariate logistic regression showed that carriers of the E4 allele(E3/E4+E4/E4 genotypes)were significantly association with hypertriglyceridemia(OR=2.561,95%CI=1.243-5.277,P=0.011),when the E3/E3 genotype was set as the reference group.(3)There were no statistical difference in fasting blood glucose and glycated hemoglobin among patients with different Apo E genotypes,but there was a significant difference in randomized serum uric acid levels(P=0.002).The Apo E-E2 carriers hadsignificantly higher serum uric acid compared with E3 homozygotes and Apo E-E4 carriers.Multivariate logistic regression analysis found that setting E3/E3 genotype as reference group,the carriers of E2 allele(E2/E2+E2/E3genotypes)were also significantly associated with hyperuricemia(OR=2.323,95% CI=1.316-4.10,P=0.004)after adjusting for sex,alcohol,smoking,TC,TG,Cr and BUN.CONCLUSIONS: Apo E polymorphisms are associated with lipid and serum uric acid metabolism in patients with hypertension and/or coronary heart disease.Compared with E3 homozygotes(wild-type),Apo E-E4 allele was associated with higher triglycerides,and the E2 allele was associated with elevated serum uric acid levels.
