| Background:With the acceleration of the aging process of the Chinese population, the continuous improvement of people’s living standards, the formation of poor eating habits, the number of patients with coronary heart disease is increasing, and the mortality rate among them is at the forefront. The burden of effective prevention and treatment of coronary heart disease is of great significance at the moment.Tobacco exposure, blood pressure, blood glucose, and lipid levels are the main predictors of coronary heart disease risk assessment. However, these factors are susceptible to a variety of factors such as antihypertensive drugs, hypoglycemic drugs,lipid-lowering drugs, and lifestyle, causing large deviations in the predicted results. Therefore,a lot of research has been done at home and abroad to find more effective and stable indicators for predicting coronary artery disease. These new predictors include serum cathepsin K(CatK),lipoprotein-associated phospholipase A2(LP-PLA2),ischemia modified albumin(IMA),and apolipoprotein(APOE),etc.Among them,APOE is simple to detect, quantified and has good stability, and it is a number of indicators for coronary heart disease prediction.Shows great advantagesObjective:To study the correlation between the APOE gene phenotype and the severity of coronary artery disease, and to provide relevant evidence for clinical intervention to control the progression of coronary artery disease in patients with coronary heart disease. Methods:A total of 583 patients who underwent coronary angiography at Zhujiang Hospital from August 2018 to May 2019 were selected. The degree of coronary lesions was evaluated using the GENSINI score,With the third quartile as the boundary, patients with coronary heart disease were divided into three subgroups with different degree of disease:mild lesion group(83 cases),moderate lesion group(83 cases),and severe lesion group(81 cases).collect clinical data and relevant blood biochemical results. The proteins encoded by different genotypes of APOE are divided into three groups of gene phenotypes:E2 gene phenotype group(E2/E2,E2/E3),E3 gene phenotype group(E3/E3,E2/E4),and E4 gene phenotype group(E3/E4,E4/E4).Univariate and multi-group comparisons were used to analyze the three groups of gender, age(age),creatinine(Cr),alanine aminotransferase(AST),glycated hemoglobin(HbA1c%),and homocysteine(HCY).,Triacylglycerol(TG),high-density lipoprotein(HDL),low-density lipoprotein(LDL),total cholesterol(TC),GENSINI score and other inspection indicators;and the use of multiple ordered logistic regression analysis of APOE gene phenotype and GENSINI score RelationshipResult:2.1 In patients with non-coronary heart disease, there were no significant differences in the different APOE gene phenotypes in age,Cr,AST,HbA1c%,HCY,TG,HDL,GENSINI score,LDL,TC(P>0.05)2.2 Among patients with coronary heart disease, there was no significant difference in the comparison of gender,age,Cr,AST,HbA1c%,HCY,TG,and HDL among different APOE gene phenotypes(P>0.05);while in GENSINI score,LDL,TC The comparison was statistically significant(P<0.05).The GENSINI score,LDL,and TC of the E4 gene phenotype group were higher than those of the E2 gene phenotype group(P<0.05),and the LDL and TC of the E4 gene phenotype group were higher than the E3 gene phenotype group(P<0.05)2.3 There were significant differences in the comparison of HDL,LDL and APOE among the three subgroups with different degrees of coronary artery disease(P<0.05).2.4 HDL and APOE were closely related to GENSINI score by multiple ordered logistic regression test(P<0.05).Conclusion:1.In patients with non-coronary heart disease, the APOE gene phenotype has no correlation with the severity of coronary angiopathy2.In patients with coronary heart disease, the APOE gene phenotype is associated with the severity of coronary vascular disease, and E2 and E3 are less severe than E4 gene phenotype. |
