Background and purposePostoperative gastroparesis(postoperative gastroparesis,PGS)is also known as postoperative gastric emptying disorder,which is common in the resection of gastric cancer after large resection of gastric cancer.The incidence of PGS in gastric cancer after partial resection of gastric and gastric cancer is about 5%-15%.In PGS patients,weight loss and malnutrition can be perceived to be a serious threat to the quality of diet and quality of life in patients with.At present,there are few studies on the etiology of PGS at home and abroad.There is a lack of systematic research on large sample studies,which leads to great divergence in the current clinical risk factors for PGS after gastric cancer surgery.In view of this,this study analyzed the correlation between the clinical data of gastric cancer patients and the occurrence of PGS after gastric cancer surgery,and analyzed the risk factors of PGS after gastric cancer surgery,in order to provide a theoretical reference for PGS clinical prevention and treatment.research purposeMethods a total of 85 cases of gastrectomy for gastrectomy in the Department of surgery,Affiliated Hospital of Fujian Medical University from January 2015 to December January 2015,were collected and divided into non PGS group and PGS group according to whether there was postoperative-2017.Using the data of preoperative baseline(such as gender,age,history of hypertension,and mouth obstruction,adjuvant chemotherapy,malnutrition,anemia),intraoperative(bleeding,gastrointestinal anastomosis,operation time intraoperative and postoperative data)(postoperative analgesia pump application,postoperative complications,anxiety after surgery,pathological staging)were observed,do statistical analysis by SPSS20.0 software.Single factor analysis of the observed correlation between the index and the incidence of PGS in patients with gastric cancer after operation,the single factor analysis of the statistically significant factors into the non conditional Logistic regression model,finally obtained the protection of PGS risk factors and independent risk factors.Research resultsThere were 10 cases of PGS after operation in 85 cases,and the incidence of PGS was 11.76%(10/85)after operation.The results of univariate analysis showed that obesity(X~2=11.506,P=0.003<0.05),preoperative malnutrition(X~2=7.824,P=0.011<0.05),preoperative pyloric obstruction(X~2=9.892,P=0.009<0.05),preoperative blood glucose level(X~2=5.273,P=0.021<0.05),preoperative blood plasma albumin level(X~2=6.238,P=0.016<0.05),gastrointestinal anastomosis(X~2=2.198,P=0.041<0.05),postoperative complications(X~2=8.427,P=0.010<0.05),postoperativeanxiety(X~2=1.888,P=0.044<0.05)andtheoccurrenceof postoperative gastric cancer is closely related to PGS.Non conditional Logistic regression analysis showed that:obesity(OR=0.651,P=0.065>0.05),preoperative malnutrition(OR=0.484,P=0.091>0.05),gastrointestinal anastomosis(OR=2.595,P=0.087>0.05),postoperative complications(OR=1.526,P=0.069>0.05)for postoperative gastric cancer risk factors of PGS protection,and preoperative pyloric obstruction(OR=0.815,P=0.016<0.05)preoperative blood glucose,water(OR=3.656,P=0.002<0.05),preoperative albumin level(OR=2.788,P=0.008<0.05),postoperative anxiety(OR=1.156,P=0.011<0.05)for postoperative gastric cancer PGS independent risk factors.research conclusionThe pathogenesis of PGS is not clear.The domestic and foreign literature study shows that the occurrence of PGS after the operation of gastric cancer is the result of many factors.Obesity,preoperative malnutrition,gastrointestinal anastomosis and postoperative complications were risk factors for PGS after gastric cancer surgery.Preoperative pyloric obstruction,preoperative blood glucose,preoperative albumin level and postoperative anxiety were independent risk factors for postoperative PGS.We should pay close attention to obesity,malnutrition,preoperative gastrointestinal anastomosis,postoperative complications and the incidence of PGS in patients with gastric cancer patients after gastric cancer surgery,should focus on the preoperative pyloric obstruction,preoperative blood sugar water,preoperative albumin level,postoperative anxiety status of gastric cancer patients after the operation of PGS risk factors,and to take specific measures to reduce the postoperative patients with gastric cancer incidence of PGS in patients with gastric cancer after surgery,promote early rehabilitation,improve the life quality of the patients.
