Pulpal and periapical disease is a kind of inflammatory disease caused by bacterial infection, which occurs in the pulp and periapical tissues. It is a common and frequently-occurring disease of oral cavity and also one of the main causes of toothache.Root canal therapy is currently the most effective treatment for pulpal and periapical disease. There are many factors that influence the success of root canal therapy, and root canal leakage is often one of the main reasons for the failure of root canal therapy. The variability and complexity of permanent root canal system is important factor leading to root canal leakage. In the past several decades, there are many encouraging advances in endodontics such as the promotion of DOM;ultrasonic instruments used in root canal treatment;the applications of CBCT.Due to these revolutionary progress, the long-term success rate of root canal therapy is greatly improved, and the result of root canal therapy is more predictable. In this paper, several cases of the treatment of variant root canal were reported, aiming to explore the operation points of CBCT-Aided Microscopic and Ultrasonic Treatment for Variant root canal. Then we make a preliminary evaluation on the efficacy of the treatments, and help to accumulate clinical experience.
