Objective:To to explore the mutation characteristics,correlation and clinical value of apoptosis related genes Bcl-2,c-myc and Survivin in gallbladder cancer group,gallbladder stone group,gallbladder polyp group and healthy person group,so as to provide a new way for early warning,early diagnosis and treatment of gallbladder cancer.Methods:Gather specimens of 44 cases after resection of gallbladder and 20 cases of gallstones in the gallbladder tissue,20 cases of gallbladder polyps group,30 cases of healthy blood specimens,the medical records and complete the relevant patient follow-up.By pathological section of gallbladder carcinoma,gallbladder tissue in gallbladder stones and gallbladder polyps,pathological diagnosis was made one by one under light microscope to observe pathological features such as histological differentiation,pathological type,peripheral organ invasion,and metastasis.The primary pathological tissue waxes of the patients entering the stockpile into the group were retrieved,re-sliced,produced,extracted DNA,amplified,and sequenced.Healthy people extracted the blood and measured the exon mutation of three genes in the blood as a control.Detection of gene mutations: Extract gallbladder paraffin specimens of DNA,gallstones in the gallbladder tissue and gallbladder polyps paraffin specimens of DNA,healthy blood samples extracted DNA,design the above three kinds of gene exon sequence primers,for quantitative determination of genome amplification reaction was achieved using polymerase chain reaction technology,and to use amplification products for purification and sequencing analysis,observation of the bcl-2 and c myc and survivin gene mutation in exon.Testing the bcl-2,c-myc and survivin mutation characteristics,analyzes its with age,sex,type of organization,the degree of differentiation,TNM staging,lymph node metastasis,such as the relationship between the clinical features,and discusses three kinds of gene mutations in the gallbladder,gallbladder stone group,the relevance of the gallbladder polyps group and healthy group.Results:The mutation rate of apoptosis inhibitor gene Bcl-2 in gallbladder carcinoma tissue was significantly higher than that in gallbladder stone group,gallbladder polyp group and healthy person group,with statistical significance(P< 0.05),and Bcl-2 was weakly correlated with cholecystolithiasis,and statistical analysis(r=0.4 P<0.05).The mutation rate of c-myc in the carcinoma of gallbladder was significantly higher than that in the gallstone group,the group of gallbladder polyps and the group of healthy persons,and there was a statistical significance(P< 0.05).The mutation rate of Bcl-2 and c-myc genes in gallbladder cancer was significantly higher than that of survivin mutation,which was statistically significant(P< 0.001).There was a weak correlation between Bcl-2 and Survivin,c-myc and Survivin(?=0.1775,0.1283).There was no significant difference in Survivin mutation rate between gallbladder carcinoma group and gallbladder polyp group(P > 0.05),which was statistically significant compared with healthy group(P< 0.05).The survivin gene was low mutation in the carcinoma of the gallbladder,and a significant mutation was found in the healthy human group.The incidence of primary gallbladder cancer was predicted by Bcl-2,c-myc and Survivin,and the accuracy rate was 97.3%.At the same time,it is also suggested that the mutation of the three groups in the carcinoma of gallbladder is not related to age,sex,TNM staging,degree of differentiation,lymphatic metastasis and so on.Conclusion:1.The exon mutation rate of Bcl-2,c-myc and Survivin genes is higher in the process of gallbladder cancer,which suggests that it is involved in the formation of gallbladder cancer.2.There is a certain correlation between bcl-2 expression and cholecystolithiasis;Bcl-2 has a high mutation rate in adenocarcinoma.There is no conclusion that the expression of bcl-2,c-myc and survivin genes in gallbladder carcinoma is related to age,sex,TNM stage,differentiation,and lymphatic metastasis.3.The Bcl-2,c-myc and Survivin genes have certain value in the clinical diagnosis of primary gallbladder carcinoma.
