Objects:Based on evaluating the influence of acupuncture with/without deqi on the symptoms and quality of life(QoL)in patients with Functional Dyspepsia(FD),detecting the changes in the plasma levels of Leptin(LP)and Soluble Leptin Receptor(SLR)of FD patients induced by acupuncture with/without deqi sensation so as to explore the mechanism of deqi influencing the gastrointestinal modulating effect of acupuncture treatment.Methods:Fivety-two FD patients were included in this study and randomly divided into two groups: expecting deqi group and not expecting deqi group.Both groups chose zhongwan(RN12)and Zusanli(ST36)as the acupuncture prescription.Patients in each group accepted manual acupuncture treatment 1 time per day and 20 times in total.The MGH Acupuncture Sensation Scale(MASS)and Visual Analogue Scale(VAS)were selected to evaluate the real needling sensation of patients each time after acupuncture treatment.According to the needling sensation,the patients were divided into the real deqi group and the real without deqi group.The Leeds Dyspepsia Questionnaire(LDQ)and Nepean Dyspepsia Life Quality Index(NDLQI)were used to evaluate the improvement of symptoms and QoL after acupuncture treatment in two groups.Enzyme Linked Immunosorbent Assay(ELISA)technique was used to detect the concentration changes of LP and SLR in plasma levels of FD patients after acupuncture treatment in two groups.All data were analyzed by SPSS22.0 software.Results:1.Fivety-two patients had finished this study.Twenty-five patients were divided into the real deqi group and twenty-five patients were divided into the real without deqi group,and two patients were not included in statistical analysis for uncertain deqi sensation.There were no statistical differences in baseline data of the two groups(P>0.05).2.The influence of acupuncture with/without deqi on the symptoms and QoL in FD patients:(1)Dyspepsia symptoms: the within-group analysis showed that the LDQ scores of FD patients in both groups were decreased significantly after acupuncture treatment(P<0.05);After treatment,the scores of the four major symptoms(postprandial fullness discomfort,early satiety,epigastric pain,epigastric burning sensation)in deqi group were significantly lower than that before treatment(P<0.05);The scores of postprandial fullness discomfort,early satiety,epigastric burning sensation in without deqi group were lower than that before treatment(P<0.05),while the decrease of the score of epigastric pain was not statistically significant(P>0.05)in without deqi group.The between-group analysis showed that there was no significant difference in the decrease of the LDQ scores and the scores of the four major symptoms between the two groups(P>0.05).(2)Quality of life: the within-group analysis showed that the average score of NDLQI in both groups improved significantly after acupuncture treatment(P<0.05);After treatment,the scores of the four fields(interference field,control field,food and beverage filed,sleep filed)in both groups improved significantly than that before treatment(P<0.05).The between-group analysis showed that there were significant differences in the improvement of NDLQI score between the deqi group and the without deqi group(P<0.05).Meanwhile only the improvement of NDLQI in deqi group was valuable in clinical(>10 points).The improvement of interference field in deqi group was higher than that in without deqi group(P<0.05),and there was no significant difference in the improvements of the score of control field,food and beverage field,sleep field between the two groups(P>0.05).3.The influence of acupuncture with/without deqi on the plasma levels of LP and SLR in FD patients: the within-group analysis showed that the plasma levels of LP of FD patients in both groups improved significantly after acupuncture treatment(P<0.05);After treatment,the plasma levels of SLR in deqi group were significantly lower than that before treatment(P<0.05),and there was no significant difference in decreasing the plasma level of SLR in without deqi group(P>0.05).The between-group analysis showed that there was no significant difference in the changes of the LP and SLR levels between the two groups(P>0.05).Conclusions:1.Acupuncture with/without deqi significantly improve the symptoms of FD patients.2.Compared to acupuncture without deqi,acupuncture with deqi improves the Qo L of FD patients significantly,and the improvement in Qo L in deqi group is clinically valuable.3.Both acupuncture with/without deqi improve the LP level in plasma of FD patients;Acupuncture with deqi decreases the SLR level in plasma of FD patients.4.Acupuncture with deqi improves the symptoms and Qo L of FD patients significantly may be related to the regulation of plasma levels of LP and SLR.
