Objective:To observe the influence of needling the He-Mu-point combination on the LP(Leptin)and SLR(Soluble Leptin Receptor)levels in plasma of Functional Dyspepsia(FD)patients by comparing needling at Lower-He point alone,so as to partly explore the mechanism of acupuncture at He-Mu-point combination regulating the gastrointestinal function.Methods:This study included 40 patients,which were randomly divided into two groups:He-Mu-point combination group(needling at ST36 and RN12)and single Lower-He point group(needling at ST36 alone).Manual acupunture treatment was performed at these points respectively once daily and totally for 20 days.The Symptom index of dyspepsia(SID)and Nepean dyspepsia life quality index(NDLQI)were used to evaluate the clinical efficacy before and after acupuncture treatment in two groups.Ensyme-linked ImmunoSorbent Assay(ELISA)technique was used to detect the concentration of LP and SLR in the plasma of FD patients before and after acupuncture in each group.Results1.There were no statistical differences in baseline data of the two groups(P>0.05).2.The clinical efficacy evaluation of acupuncture for FD patients in the two groups.(1)The SID tatal score of the both two groups after acupuncture treatment decreased significantly compared to those at baseline(P<0.05),Acupunture at He-Mu-point combination could significantly lower the four main symptoms score(P<0.05),while acupuncture at Lower-He point alone could distinctively improve the symptoms of "postprandial fullness"(P<0.05)and "epigastric pain"(P<0.05).No statistical differences in the improvement of SID scores were found between the two groups(P>0.05).(2)The NDLQI scores of the both two groups after acupuncture treatment were increased significantly compared to those at baseline(P<0.05).However,in the acupuncture at He-Mu-point combination group,the improvement of NDIOL score after treatment had clinical significance(MCID≧ 10).Acupuncture at He-Mu-point combination could significantly improved the Individual and average scores of NDLQI(P<0.05),while acupuncture at Lower-He point alone could significantly increase the average,interference field,cognitive control field,and food and drink field score(P<0.05).The statistical differences in the improvement of the interference field,cognitive control field,and sleep field score were found between the two groups(P<0.05).3.The influence of acupuncture on SP and SLR in plasma level for FD patients in two groups.(1)The within goup analysis:acupuncture at He-Mu-point combination significantly improve the LP level in plasma(P<0.05),and lower the SLR level in plasma(P<0.05).Acupuncture at Lower-He point alone significantly improve the SLR in plasma level(P<0.05),but had no obvious influence on the LP in plasma of FD patients(P>0.05).(2)The between-goup analysis:there was significant statistical difference in changes of plasma level of SLR between the two groups(P<0.05),but no statistical difference in the improve LP level in plasma(P>0.05).4.The correlation between the improvement of LP and SLR levels in plasma and clinical efficacyThe increased LP level in plasma and decreased SLR level in plasma at He-Mu-point combination were significantly correlated with the decrease of the“epigastric pain" symptom score(P<0.05).Conclusion1.Both acupuncture at He-Mu-point combination and at Lower-He point alone can improve the symptoms and life of quality of FD patients,but He-Mu-point combination has comprehensively clinical effect,and the improvement of NDIOL score after treatment had clinical significance than before acupuncture.The changes in the interference field,cognitive control field,and sleep field scores in He-Mu-point combination group are higher than those in Lower-He point group.2.Acupuncture at He-Mu-point combination significantly increase the LP level,and lower the SLR level in plasma.Acupuncture at Lower-He point alone only improve the SLR level in plasma.The raised plasma level of LP and decreased plasma level of SLR in He-Mu-point combination group were significantly correlated with the decrease of“epigastric pain" symptom score3.The regulation of LP and SLR level might be probably one of the important mechanism of acupuncture at He-Mu-point combination treating for FD. |
