Serum Endoglin Level And Cardiovascular Risk Factors In Communities Middle-Aged And Elderly Population

ObjectivesTo investigate the serum endoglin level in four communities in Fujian Province,and to explore the association of serum endoglin level with blood pressure,lipid profiles,blood glucose and carotid intima medium thickness?CIMT?.MethodsA resident group of 983 subjects aged 40 years or older was selected for a cross-sectional survey according to the inclusion and exclusion criteria from May2016 to May 2017 in four communities in Fujian Province.Demographic data collection,physical examination,laboratory examination,carotid ultrasound examination and the serum endoglin detection were conducted.One-way analysis of variance was used to compare the general clinical characteristics among different groups of serum endoglin level.The multiple linear regression analysis was applied to explore the correlation of serum endoglin level with blood pressure,lipid profiles and blood glucose.The binary logistic regression analysis was applied to analyze the risk factors of hypertension,hyperlipidemia,diabetes and increase in CIMT.Results1.A total of 983 subjects were selected for the study,including 382 men and 601women,with the average age of 57.3±9.7 years and male female ratio of 1:1.6.The prevalence of hypertension,diabetes,dyslipidemia and CIMT thickening in the total population was 47%,17.9%,67%and 29.1%,respectively.2.The average serum endoglin level was 12.52?9.77,15.79?ng/ml in the total population.The age,systolic pressure,diastolic pressure,triacylglycerol,total cholesterol,low density lipoprotein cholesterol?LDL-C?,fasting blood glucose,hemoglobin A1c?HbA1c?,uric acid,serum creatinine,CIMT,and the proportion of smoking and lack of exercise were increased with the elevated Ln?endoglin?level in quartile groups?all P<0.05?.3.The level of serum endoglin was positively correlated with age,body mass index?BMI?,systolic pressure,diastolic blood pressure,triacylglycerol,total cholesterol,LDL-C,fasting blood glucose,HbA1c,uric acid,creatinine and CIMT,and was negatively correlated correlated with high density lipoprotein cholesterol?HDL-C??all P<0.05?.4.Multiple linear regression analysis showed that after adjustment for age,smoking,alcohol drinking,lack of exercise,waist circumference,BMI and other factors,endoglin level had a linear positive correlation with systolic pressure,diastolic pressure,triacylglycerol,total cholesterol,LDL-C and HbA_1c,and the regression coefficient was 0.404?95%CI:1.379-2.762?,0.311?95%CI:1.764-2.604?,0.145?95%CI:0.147-0.877?,0.173?95%CI:0.253-0.726?,0.095?95%CI:0.033-0.539?and 0.110?95%CI:0.080-0.580?,respectively.5.The binary logistic regression analysis showed that the odds ratio?OR?values of hypertension in the third and highest quartile of serum endoglin were 2.907?95%CI:1.681-5.025?and 7.286?95%CI:4.161-12.756?respectively in comparison with that in the lowest one.The OR values of dislipidemia in the third and highest quartile of serum endoglin were 6.789?95%CI:3.549-12.988?,8.061?95%CI:4.134-15.716?respectively in comparison with that in the lowest one.The OR value of diabetes in the highest quartile of serum endoglin was 2.304?95%CI:1.147-4.629?compared with that in the lowest one.6.Multiple linear regression analysis showed that after adjustment for age,sex,smoking,alcohol drinking,lack of exercise,waist circumference,BMI and other factors,endoglin level had a linear positive correlation with CIMT,with the regression coefficient of 0.206?95%CI:0.125-0.198?.Age,smoking,BMI,systolic blood pressure,total cholesterol and uric acid were positively correlated with CIMT,the regression coefficients were 0.312?95%CI:0.004-0.007?,0.090?95%CI:0.005-0.071?,0.094?95%CI:0.001-0.012?,0.112?95%CI:0.001-0.002?,0.061?95%CI:0.005-0.025?and 0.071?95%CI:0.001-0.002?,respectively.7.The binary logistic regression analysis showed that age,smoking,BMI,systolic blood pressure,total cholesterol,uric acid and endoglin were the risk factors for the increase in CIMT.The OR values of increased CIMT in the third and highest quartile of serum endoglin were 5.342?95%CI:2.149-13.275?and 13.431?95%CI:5.468-32.991?respectively in comparison with that in the lowest one.ConclusionThe average level of serum endoglin was 12.52?9.77,15.79?ng/ml in communities middle-aged and elderly population.The serum endoglin level was closely related to the cardiovascular risk factors such as blood pressure,blood glucose and lipid profiles.Age,smoking,BMI,systolic blood pressure,total cholesterol,uric acid and serum endoglin level were closely related to the carotid atherosclerosis in the study.Serum endoglin is expected to be a new biomarker for evaluating cardiovascular diseases and subclinical atherosclerosis.
