Purpose:In the study,the menstrual migraine patients as the research object,and the clinical efficacy of acupuncture in patients with menstrual migraine was observed.The fMRI technique was used to investigate the abnormalities of precuneus function network in patients with menstrual migraine from the perspective of neuroimaging.At the same time,expore the menstrual migraine patients with precuneus function connection network;exploring the central nervous mechanism of acupuncture treatment for patients with menstrual migraine to provide adequate scientific basis and promotewidely for prevention and treatment Method:In the study,31 cases of menstrual migraine patients as the object of the study and compared with 24 healthy volunteers,to study the changes of brain functional connectivity of precuneus in patients with migraine,then randomize divided into acupoints and non-acupoints,respectively for 3 menstrual cycles in two groups of patients with menstrual migraine,and 27 times treatment,and before treatment,treatment for 4 weeks,8 weeks,12 weeks were evaluated,and before and after12 weeks treatment were given patients fMRI to observe the effect of acupuncture on brain functional connecting of patients with menstrual migraine to explore the mechanism of acupuncture treatment ofneuroimaging.Result:1.After the acupuncture treatment of menstrual migraine,headache attacks,VAS,headache intensity,HIT-6 are significantly improved before treatment;acupoints in patients with headache frequency better than non acupoint(P <0.05).there were significant differences between the two groups in the frequence of headache and HIT-6 after eighth weeks treatment and there is no significant difference between the two groups in other indexs(P> 0.05).2.Compared with healthy subjects,Patients of menstrual migraine,precuneus functional connectivity is decreased with Bilateral inferior frontal gyrus,bilateral medial frontal gyrus,bilateral middle frontal gyrus,bilateral superior frontal gyrus,bilateral anterior cingulate,bilateral parahippocampal gyrus,bilateral posterior cingulate gyrus,bilateral thalamus,bilateral cuneus,occipital gyrus,bilateral temporal gyrus,bilateral superior temporal gyrus,bilateral cerebellar tonsil,bilateral inferior semilunar lobule,right caudate nucleus,right lentiform nucleus,right insula,right fusiform gyrus,right side slopesand so on,and have not found functional connectivity enhanced..3.Compared with before treatment,after treatment of acupoint group,precuneus functional connectivity is increased with Bilateral inferior frontal gyrus,bilateral medial frontal gyrus,bilateral middle frontal gyrus,bilateral precentral gyrus,bilateral superior frontal gyrus,bilateral anterior cingulate,right caudate right putamen,bilateral insula,right lentiform nucleus,right inferior parietal lobule,right postcentral gyrus,bilateral temporal gyrus,and precuneus functional connectivity is decreased with Bilateral inferior parietal lobule,left middle temporal gyrus,right inferior frontal gyrus,right middle frontal gyrus,right medial frontal gyrus.after non acupoint group treatment,precuneus functional connectivity is increased withRight middle frontal gyrus,left medial frontal gyrus,right superior frontal gyrus,precuneus functional connectivity is decreased with Bilateral medial frontal gyrus,bilateral middle frontal gyrus,bilateral paracentral lobule,bilateral precentral gyrus,bilateral superior frontal gyrus,bilateral parahippocampal gyrus,bilateral posterior cingulate gyrus,bilateral occipital gyrus,right postcentral gyrus,bilateral temporal gyrus,bilateral temporal gyrus.4.the change of precuneus with MFG functional connectivity after acupuncture with VAS is negatively correlated(P<0.01),and precuneus with caudate functional connectivity with VAS is negatively correlated(P<0.01),however,acupuncture non acupointonthe evaluation of the efficacy of indicators have not found the corresponding response in the central nervous.Conclusion:1.Through the clinical observation of acupuncture treatment of patients with menstrual migraine,after treatment the menstrual migraine patients headache frequence,pain intensity,VAS and other clinical indicators were significantly improved compared with before,which shows that acupuncture on the Menstrual migraine is effective,acupuncture acupoints in headache frequence is better than acupuncture non acupoint.2.Choosing the precuneus as a region of interest for functional connectivity analysis found that,compared with healthy subjects,menstrual migraine patients precuneus function connectivity decreased with DMN which mainly obtain the precuneus,Temporallobe,hippocampus,etc.which also includes the parietal and frontal lobe of the executing function network,there is also the basal ganglia region such as caudate nucleus and lenticular nucleus,these abnormal brain functions may be the central pathological factors of headache in patients with menstrual migraine.3.The precuneus and MFG function connectivity is enhanced,which may be related to the cognitive function of the patients with menstrual migraine,and the increased of the precuneus and cauduate function connectivity may be related to the improvement of intensity of pain.
