Clinical Analysis Of Early-onset And Late-onset Of Hospitalized Patients Of Psoriasis

ObjectiveConsulting and classifing the medical records and tests of our in-patients in dermatological department to understand the difference of clinical and epidemiologic characteristics and possible complications between early-onset psoriasis and late-onset psoriasis.MethodsUsing epidemiologic method to registered the clinical cases and laboratory examinations of the patients who were treated in dermatological department from January 2015 to November 2017,established the database by EXCEL 2007 software and analyzed by SPSS19.0 software.Results1.This study included 547 cases,of which 355 were male,192 were female,male to female ratio was 1.85:1.The average age of patients was 47.3±16.572 years,and the mean age of frist onset was about 33.12±15.547 years old.There was no difference of frist onset age between male and female.And only one peak could be seen in the frist onset age curve.2.The EOP group had 369 cases,accounting for 67.5%,and the LOP group had178 cases,accounting for 32.5%,the EOP patients was more.More male patients were in Both EOP and LOP group than female(p>0.05).The onset of the EOP or the LOP was unrelated to gender(p>0.05).And there was no statistically significant difference in clinical phenotypes and nail involvement between the two groups(p>0.05).Patients with EOP had a longer course of illness.Patients with positive family history were onset earlier than those with negative family history(p<0.05),and the positive rate of family history was higher in EOP group.There was no statistically significant difference between male and female family history both in EOP and LOP groups(p>0.05).In terms of personal histories,there was no statistically significant difference between the EOP group and the LOP group(p>0.05),but the LOP group had more alcohol history(p<0.05).In terms of treatments,LOP patients were more treated with system drugs than EOP group(p<0.05).3.The LOP group had a higher average BMI value,and easier to have overweight or obesity.Patients with LOP were 1.791 times more likely to be overweight or obesity than those with EOP(OR=1.791,95%CI:1.248~2.571).In EOP group,the BMI value was positively correlated with the onset age(r=0.116,p=0.026).But there was no obvious correlation between the BMI value and the first onset age of the LOP patients(r=0.067,p=0.378).There was no significant difference between EOP and LOP groups with blood lipid disorder(p>0.05),but there were more patients with hyperglycemia,hypertension and coronary heart disease in LOP group(p<0.05).Conclusion1.The first onset age of psoriasis was no difference between male and female.There was no obvious for bimodality in age of onset of psoriasis.2.The pathogenesis of psoriasis is related to genetic factors.Patients with positive family history could onset earlier,and the patients with EOP were more affected by genetic factors than those with LOP.3.Patients with LOP were more likely to be overweight or obesity.4.There was a positive correlation between the BMI value and the first onset age of the EOP patients.But there was no obvious correlation between the BMI value and the first onset age of the LOP patients.5.Patients with LOP were easier to have hyperglycemia,hypertension and coronary heart disease than those with EOP.
