BackgroundNowadays,coronary atherosclerotic heart disease is one of the major cardiovascular disease that threaten human health and life.The 2017 China Cardiovascular Disease Report pointed out that China currently has approximately 290 million cardiovascular patients,of which 11 million have coronary heart disease.Framingham study has confirmed a series risk factors of CHD,including age,gender,family history,smoking and so on,which called "traditional risk factors".With the development of evidence-based medicine,people have a new understanding of the risk factors of CHD,the “new and emerging risk factors ”such as blood lipids,metabolic factors,inflammation factors,psychological factors,socioeconomic status,sleep quality and oral health,play an important role in predicting CHD.This article intends to explore the traditional risk factors of coronary heart disease and some lifestyle risk factors,including oral hygiene,sleep duration,sleep snoring and the incidence of CHD.To provide the scientific basis for CHD prevention and treatment.AimAccording clinical questionnaire,to explore the relationship between traditional risk factors and poor living behaviors and coronary heart disease,the traditional risk factors including gender,age,body mass index,smoking,alcohol,hypertension,diabetes;poor living behaviors covering oral health ?sleep duration and sleep apnea.Comparng the differences of traditional risk factors and poor living behaviors between the normal coronary group,atherosclerosis group and coronary heart disease group.Identify independent risk factors for coronary heart disease.Methods1.A total of 380 patients suspected coronary heart disease were selected and performed coronary angiography,in General Hospital of Tianjin University from March 2016 to July 2017.CAG showed that 42 patients with normal coronary arteries were normal coronary arteries,78 patients showed coronary atherosclerosis coronary were the AS group,and 260 patients were diagnosed with coronary artery disease was coronary heart disease group.A total of 380 inpatients were surveyed.The questionnaire included general information such as the name,sex,age,as well as family history,smoking,drinking,income education,oral hygiene,self-reported sleep conditions,sleep snoring.2.All fasting subjects' venous blood were taken in the morning at 6am-7 am,professionals in Tianjin Medical University General Hospital examination department in the day to determine blood routine,liver and kidney function,fasting blood glucose,blood lipids,myocardial enzymes,coagulation function.Results1.There was no significant difference in age,body mass index,family history of CHD,white blood cell count,lymphocyte,creatine insase,creatine kinase isoenzyme,glutamic oxalacetic transaminase,glutamic pyruvic transaminase,total bilirubin,direct bilirubin,total holesterol,triglyceride,low density lipoprotein cholesterol in normal coronary artery group,AS group and coronary heart disease group(P>0.05).2.There was no significant difference in income and education level between normal coronary artery group,AS group and coronary heart disease group(P>0.05).3.The proportion of male,smoking,drinking,hypertension and diabetes in normal coronary artery group were higher than AS group and normal coronary group(P<0.05).The level of neutrophil,neutrophil and lymphocyte ratio,hemoglobin,creatinine,fibrinogen and blood glucose in coronary heart disease group,were significantly higher than those in AS group and normal coronary artery group,high-density lipoprotein cholesterol in coronary heart disease group and AS group were significantly lower than that in normal coronary artery group(P<0.05).4.There were statistical differences in the number of missing teeth among the three groups(P>0.05).The proportion of the number of teeth missing?10 in coronary heart disease group was significantly higher than the normal coronary group(P<0.05),although the number of patients who had lost more than 10 teeth in coronary heart disease group were higher than that in AS group,there was no statistically difference(P>0.05).Teeth loss was an risk factor of coronary heart disease.5.There was no significant difference of sleep time and sleep quality among the three groups(P>0.05).The proportion of sleep time <7 hours and sleep time >8 hours in coronary heart disease group was significantly higher than normal coronary group(P<0.05).The sleep duration<7h and sleep duration>8h were risk factor of coronary heart disease.6.There was significant difference of snoring?snoring sounds and sleep apnea among the three groups(P<0.05).The proportion of snorers in coronary heart disease group was sigificanely higher than that in normal coronary group(P<0.05).The proportion of loud snoring in coronary heart disease group was sigificanely higher than the normal coronary group(P<0.05).The proportion of sleep snore in coronary heart disease group was sigificanely higher than AS group and normal group(P<0.05).Snoring,snoring loudness and sleep apnea were risk factors of coronary heart disease.7.The results of univariate logistic regression and multivariate logistic regression analysis showed that male,hypertension and diabetes,fibrinogen,teeth missing?10,sleep time <7 hours and sleep time >8 were independent risk factors of coronary heart disease.Conclusion1.Single factor related analysis showed that,traditional risk factors: such as male,smoking,drinking,hypertension,diabetes and the higher level of neutrophil absolute,NLR,Hb,Cr,FIB,hs-CRP,FBG,and HDL-C lower were risk factors of coronary heart disease.2.The number of teeth missing ?10,the sleep duration<7h and sleep duration>8h,snoring,snoring loudness and sleep apnea were the risk factors of coronary heart disease.3.Logistic multivariate regression analysis showed that men,diabetes,hypertension,FIB,the number of missing teeth ?10,the sleep duration<7h and sleep duration>8h were independent risk factors of coronary heart disease.
