Objective By reviewing and analyzing the imaging features of MRI and MSCT in a bigger group of pancreatic cystic lesions, and comparing with pathology and clinical results, the diagnostic value of the two imaging techniques was compared. The understanding of the imaging features of the lesions was improved, and the clinical choice of the treatment scheme was provided.MethodsThe clinical data and MRI and MSCT of 86 patients with pancreatic cystic lesions from January 2015 to January 2018 were analyzed retrospectively,70cases were confirmed by operation and biopsy pathology, and the other 16 cases were diagnosed in clinical and imaging follow-up.Recording the lesion's location separately, morphology, maximum diameter, number, boundary, sac wall, separation, solid composition and wall nodules, surrounding changes, cystic components, lesions and pancreatic duct, whether the main pancreatic duct expansion. In all cases of surgical or biopsy, both MRI and MSCT were performed before operation or biopsy, and the clinical data, pathology or follow-up results of all cases were analyzed, the accuracy of MRI and MSCT in diagnosing pancreatic cystic lesions was compared, and the imaging features of cystic lesions were analyzed. The MRI and MSCT were compared in the scanning time, and the statistics methods were processed. All of the MSCT uses the German Siemens company 64 Row Spiral CT or the Dutch Philips company 256 Row Spiral CT,uses the flat sweep and the dynamic enhancement scanning. All MRI uses the Dutch Philips 1.5T Machine, used the regular MRI flat sweep sequence, the fat inhibition sequence, the diffusion weighted imaging, the inphase/outphase sequence, and the dynamic radiography enhanced scans, when necessary, adds the 3D-MRCP sweeps.ResultsAmong 86 patients with pancreatic cystic lesions, the average age was(55.5±12.0),46 were male, accounting for 53.49%,and 40 were female, accounting for 46.51%.70 cases of pancreatic cystic lesions confirmed by pathology were:the Pseudocyst were 25 cases, true cysts were 7 cases, mucinous cystic tumors in 12cases,serous cystic tumors in 5 cases, intraductal papillary mucinous tumors in 13cases,solid papilloma in 6 cases, and neuroendocrine tumors in 2 cases.16 cases with clinical, imaging and follow-up diagnosis were false cysts. The diagnostic accuracy of 70 cases of pancreatic cystic lesions diagnosed by MRI and MSCT were 94.3%and 82.9%,respectively,with high diagnostic efficiency, but MRI was better than MSCT(?~2=4.516,P(27)0.05).The examination time of MSCT was shorter than that of MRI(t=2.37,P(27)0.05).ConclusionMRI and MSCT all have high efficiency in diagnosing pancreatic cystic lesions, but MRI is generally better than MSCT.MRI and MSCT have its own advantages in morphological diagnosis.MRI is superior to MSCT in displaying cystic composition, hemorrhage, sac separation, solid composition and wall nodule, and MSCT is better than MRI in showing calcification and calcification position.
