Objective:The expression of serum vascular endothelial growth factor(VEGF)in peripheral blood of the study subjects was measured.The difference of serum VEGF expression between early breast cancer patients and healthy women was statistically significant.The expression of serum VEGF and the status of sentinel lymph nodes were studied.Correlation of pathological features,tissue p53 expression and clinical pathological features.To investigate whether the serum VEGF expression level can be used as a predictor of sentinel lymph node metastasis in patients with early breast cancer and to explore its significance in assessing the degree of malignancy,prognosis,and clinical treatment.To study the relationship between p53expression and breast cancer,and provide a new theoretical basis for endocrine therapy in breast cancer patients.Methods:From september 2016 to december 2017,283 patients with early-stage breast cancer diagnosed at the Department of Breast Surgery at the Affiliated Hospital of Chengde Medical College were selected as the experimental group.Thirty healthy women who underwent physical examination at the same time in the breast surgery clinic were selected as the control group.The general data of the study subjects(age,menstrual status)were collected.The experimental group collected complete pathological data(sentinel lymph node metastasis,tumor size,lymph node metastasis,pathological type,ER,PR,HER-2,Ki-67,EGFR,p53)The subjects were all tested for serum VEGF.According to the serum VEGF quartiles,early breast cancer patients were divided into four subgroups.Sentinel lymph node status and clinical pathological features were compared between subgroups.According to tissue p53 expression,early breast cancer patients were divided into p53-positive group and p53-negative group,and the clinical pathological features were compared between the groups.Spearman correlation analysis was used to explore the correlation between VEGF and sentinel lymph node status and pathological features,and to explore the correlation between p53 and pathological features.Results:1 The serum VEGF expression level in the experimental group was(63.99±50.78)pg/ml,which was significantly higher than that in the control group(40.57±30.16)pg/ml,and the difference was statistically significant(t=31.347,P<0.05).2 The quartiles of serum VEGF expression in the experimental group were 41.49 in the first quartile,51.63 in the second quartile,70.65 in the third quartile,and in the four groups of early-stage breast cancer patients,the sentinel lymph nodes had the first to the fourth group was 18 cases(25.4%),22 cases(31.0%),34 cases(48.6%),39 cases(54.9%),the difference was statistically significant(?~2=17.498,P<0.05);Four groups in patients with early-stage breast cancer,there were 15 cases(21.1%),20 cases(28.2%),301cases(42.9%),and 34 cases(47.9%)with lymph node metastasis in the first to fourth groups,the difference was statistically significant(?~2=14.549,P<0.05);In the four groups of early breast cancer patients,the tumor diameter was less than 2cm,and the first group to the fourth group were 25cases(35.2%),13 cases(18.3%),11 cases(15.7%),11 cases(15.5%),diameter For 2-5cm,46 cases(64.8%),58 cases(81.7%),59 cases(84.3%),and 60 cases(84.5%)respectively,the difference was statistically significant(?~2=11.343,P<0.05);In patients with early-stage breast cancer,HER-2positive expression in the first to fourth groups were 24 cases(33.8%),37cases(52.1%),36 cases(51.4%),and 43 cases(60.6%),the difference was statistically significant(?~2=10.773,P<0.05)In the four groups of early-stage breast cancer patients,the first group consisted of 22 cases of Luminal A(27.8%),35 cases of Luminal B(44.3%),9 cases of HER-2 overexpression(11.4%),and 13 cases of triple-negative(16.5%).In the second group,11cases(8.0%),39 cases(28.3%),42 cases(30.4%),46 cases(33.3%),and the third group were 9 cases(27.3%)and 8 cases(24.2%).6 cases(18.2%)and10 cases(30.3%),and the fourth group was 13 cases(31.7%),13 cases(31.7%),13 cases(31.7%),and 2 cases(4.9%).Statistical significance(?~2=21.192,P<0.05);Among the four groups of early-stage breast cancer patients,the first to fourth groups of EGFR expression were 50(70.4%),53(74.6%),49(70.0%),64(90.1%),andstatisticallysignificant(?~2=10.529,P<0.05);In the four groups of early breast cancer patients,the positive expression of p53 in the first to fourth groups were 33 cases(46.5%),33cases(47.8%),31 cases(43.1%),and 32 patients(45.1%),the difference was not statistically significant(?~2=0.101,P>0.05).3 In the P53 positive group,87 cases(67.4%)were positive for ER,42cases(32.6%)were negative for ER,p53 negative group,123 cases(79.9%)were ER positive,and 31 cases(20.1%)were ER negative.The difference was statistically significant(?~2=5.665,P<0.05);p53 positive group,PR positive 75 cases(58.1%),PR negative 54cases(41.9%),p53 negative group,PR positive 114 cases(74.0%),PR negative 40 cases(26.0%),the difference was statistically significant(?~2=7.987,P<0.05).4 The results of Spearman correlation analysis showed that serum VEGF expression and tumor size(r=0.169,P=0.002),lymph node metastasis(r=0.219,P<0.001),and sentinel lymph node metastasis(r=0.243,P<0.001)in early stage breast cancer patients.HER-2(r=0.181,P=0.001),EGFR(r=0.144,P=0.008)were positively correlated;Tissue p53 expression and ER(r=-0.141,P=0.009),PR(r=-0.168,P=0.002)were all negatively correlated.Conclusion:1 The serum VEGF expression level in early breast cancer patients was higher than that in healthy women(P<0.05).The serum VEGF expression level in early breast cancer patients was positively correlated with sentinel lymph node metastasis,and the serum VEGF expression level could be considered as early breast cancer.The predictive biomarkers of sentinel lymph node status in patients;The serum VEGF expression levels in early breast cancer patients are positively correlated with tumor size and lymph node metastasis,which can be used as an index for predicting clinical stage of breast cancer;serum VEGF levels are positively correlated with tumor tissue EGFR expression.VEGF may exert biological effects through acting on EGFR.2 There was no correlation between serum VEGF expression and tissue p53 expression in patients with early breast cancer.3 The expression of p53 was negatively correlated with the expression of ER and PR.The expression of p53 was not sensitive to endocrine therapy and was one of the indicators of poor prognosis.
