In this study,patients with diabetic constipation were studied.Based on the cognitive behavioral misconceptions and the related factors such as weak spleen and stomach and adverse psychological state,the clinical nursing intervention methods guided by the system theory were constructed.In order to alleviate the effect of these factors on diabetic patients and relieve the symptoms of constipation.This study was divided into two parts.Part one Investigation on the status quo of diabetics with constipationObjective:The self-compiled questionnaire was used to investigate the current status of the patients,to explore the cognitive behavioral misunderstanding and the negative psychological state of diabetic patients with constipation,and to provide objective evidence for futher clinical intervention.Methods:A total of 138 cases of diabetic constipation were investigated by using the self-prepared questionnaire,to know about the general information and cognitive behavior of diabetic constipation.Thus,targeted cognitive reconstruction CARDS and behavioral intervention programs were made,to prepare for the cognitive behavioral intervention in the second part of this study.Results:After examination,the overall internal consistency coefficient of the questionnaire about diabetic constipation was 0.865,and the content validity index of the scale level was 0.98.In the questionnaire survey of 138 patients with diabetic constipation,the researcher ranked all the items of the questionnaire according to the score from low to high,a total of 34 entries with lower scores were retained in the end.The average score of the items in various factors such as exercise,mental psychology,medication and diet was lower.The average score of the items related to exercise was 1.57±0.56,the average score of the items related to mental psychology was 1.72±0.45,the average score of the items related to medication was 1.78±0.80,and the average score of the items related to diet was 1.99±0.38.Conclusion:1.The self-compiled questionnaire about diabetic constipation has good reliability and validity,it is suitable for the investigation of the status quo of patients with diabetic constipation.2.In this study,patients cognitive behavioral misconceptions are focused on the exercise,mental psychology,medication and diet and so on,require more intervention.Part two Study on the effect of cognitive behavior intervention combined with the improved bear play on diabetics with constipationObjective:To explore the intervention effect of cognitive behavior intervention combined with the improved operation of bear play on diabetic constipation,and to improve defecation of diabetic constipation.Methods:A total of 73 patients with diabetic constipation who were admitted to a hospital from November 2016 to May 2017 were selected as the study subjects.A random number table method was used to divide the study subjects into the experimental group 36 cases and the control group 37 cases.The patients in the control group received the routine constipation nursing intervention in the department of Endocrinology.Beside the routine constipation nursing intervention the patients in the test group received the cognitive behavior intervention combined with the improved bear play.And the effects were observed and compared.Results:1.Before intervention,there was no statistically significant difference between the two groups in gender,age,course of heat,smoking history,cultural degree,degree of constipation and other general information.There was no significant difference in symptoms of constipation between the two groups before intervention.2.The symptoms of constipation scores in the test group were significantly lower than the control group,P< 0.05.The symptom scores of the test group in various aspects,such as the difficulty of defecation,fecal traits,defecation time,frequency of defecation,abdominal distension and defecation were significantly lower than those in the control group,P<0.05.The effective rate in the test group(88.89%)was significantly higher than the control group(54.05%),P< 0.05.Conclusion:Cognitive behavior intervention combined with the improved operation of bear play can help alleviate the symptoms of diabetes patients with complication.The intervention of this study is an effective and safe method to treat constipation of diabetes and can be used in clinical application.
