Objective:Retrospectively analysis the clinical data of 205 cases of our hospital with multiple myeloma to evaluate the association between renal dysfunction and clinical characteristics.Methods:The clinical data of 205 preliminarily diagnosed MM patients in the hematology department of affiliated Hospital of Zunyi Medical College from January 2010to November 2010 were analyzed retrospectively.Collect the data about the patient's general information?age,sex?,laboratory examination?Hemoglobin,creatinine,uric acid,blood calcium,blood phosphorus,albumin,globulin,?2-microglobulin,etc.?and Imaging data.On the basis of creatinine in 205 Cases of MM patients,define creatinine<176.8umol/L as A group with normal renal function group,define creatinine?176.8umol/L as B group with abnormal renal function group.Single factor analysis of the relationship between renal function and other factors,the factors with significant difference were analyzed by logistic multifactor regression,compare the characteristics of patients between Group A and Group B to analyze the risk factors.Results:The incidence ratio of male and female was 1.28:1,with an average age of60.77years.Patients with D-S stage?accounted for 78.1%,patients with ISS stage?,?accounted for 90%,IgG MM accounted for 43.9%,light chain MM accounted for 34.15%;30.2%of the patients with creatinine was 176.8umol/L;Abnormal renal function group and normal group respectively with hemoglobin,serum calcium and phosphorus,uric acid,lactate dehydrogenase,?2-microglobulin,proteinuria,Durie-Salmon stage,ISS stage,MM type comparison,P<0.05,by statistical differences,and age,gender,ESR contrast P>0.05,no significant difference.Multivariate Logistic regression analysis results in the significant P=0.009,P<0.05 of?2-globulin,which can significantly affect whether renal dysfunction is associated.Conclusion:1.The majority of MM patients in our hospital are patients with IgG and free light chain types.2.Most patients have been in stage?or stage?before being diagnosed as MM.3.The incidence of renal impairment in light chain type is higher than that of IgG and IgA types.4.The increase of?2-microglobulin is an independent risk factor for renal dysfunction,which is of great significance for the early detection of renal dysfunction in patients with MM.
