| Objective: To explore the anatomical characteristics and application value of this approach by studying the application of Lateral supraorbital approach(LSOA)in microsurgical treatment of anterior circulation aneurysm.Methods: A retrospective analysis was made of 39 cases of anterior circulation aneurysms in our hospital were treated by Pterional approach(PA)from October 2015 to March 2018.At the same time,a total of 28 patients with anterior circulation aneurysms were treated by Lateral supraorbital approach(LSOA),and the differences between the two groups were excluded: gender,age,preoperative Hunt-Hess classification and aneurysm imaging characteristics.By recording analysis of two kinds of patients with surgical approach,intraoperative incision length cranial,aneurysm rupture rate and amount of blood transfusion,Time for craniotomy and close cranial cavity(from anatomic brain pool to successful clipping),the time used to process aneurysms during surgery,length of hospital stay,hospital recovery,the injury rate of facial nerve is lower,reducing the risk of postoperative facial nerve dysfunction,the difference between evaluation LSOA application value in the former circulation aneurysm clipping.Results: in the two groups,the rupture rate of aneurysm and the amount of blood transfusions,the time used to process aneurysms during surgery(from anatomic brain pool to successful clipping),and the recovery of patients after statistically analyzed,and p>0.05 was not statistically significant.The length of surgical incision,Time for craniotomy and close cranial cavity,the time used to process aneurysms during surgery,length of hospital stay were statistically analyzed in the two groups,and p <0.05 was considered statistically significant.Conclusion: the LSOA group has shorter incision and less trauma than the PA group.The operation is shorter,faster and less time-consuming.The patient is hospitalized shortening,the patient recovers quickly,reducing the patient’s financial burden.The injury rate of facial nerve is lower,reducing the risk of postoperative facial nerve dysfunction.LSOA group compared with PA,intraoperative aneurysm rupture rate and amount of blood transfusion,intraoperative aneurysm used time has no obvious difference,suggesting LSOA in minimally invasive,and did not increase the risk of aneurysm rupture and the difficulty of the operation.There was no difference in postoperative recovery between the two groups,indicating that LSOA did not increase the postoperative complications and other adverse effects.LSOA is more minimally invasive,faster,more economical and safer worthy of clinical promotion in the treatment of anterior circulation aneurysm. |
