OBJECTIVE:Analyse clinical characteristics of pediatric primary liver tumors retrospectivly and improve the level of clinical diagnosis and treatment.METHODS: 22 cases with primary liver tumors were collected from our hospital General surgery department and confirmed by pathology.Tumor types,age,gender distribution,laboratory tests,imaging examination,pathological examination and prognosis were summarized and analysed.RESULTS: There were 13 females(59.1%)and 9 males(40.9%)of all patients.And the median initial diagnosis age was 7 months(20 months-5 years old);13 cases(59.1%)had benign tumors and 9(40.9%)had malignant tumors.Of all the cases,one case had diaphragm metastasis and the other 8 cases were confined to the liver.4cases with congenital heart disease,the rest were not associated with other diseases;Clinical manifestations included abdominal bulging,abdominal mass,abdominal pain,vomiting and so on,of which the most common symptoms was abdominal bulging.The benign and malignant liver tumors in this group had no significant difference in RBC,HB,AFP,ALT,AST and TBA,but there were significant differences in WBC,LDH,PLT and TB.The sensitivity of imaging diagnosis with enhanced CT or MRI was 86.4%,plain CT77.2%,B-ultrasound 63.6%.The disease-free survival rate of benign liver tumors patients was 100% with surgical treatment.Malignant liver tumors were treated by surgical and chemotherapy combined treatment,and the survival rate was 66.7%.CONCLUSION: More than 90% of primary pediatric liver tumors were found within2 years old.Ultrasound was best method for screening tumors.CT or MRI was a vital method of differential diagnosis.As a tumor marker,Serum AFP in children over 6months is a major identification between benign and malignant tumor.Benign liver tumors mainly therapy was surgical resection and the most important treatment of hepatoblastoma was combination of surgery and chemotherapy.
