Object: Through detection of serum KL-6 levels of patients with PM/DM-ILD,compared with PM/DM without ILD group(PM/DM-NILD)and normal control group(NC)differences,to explore the significance of the disease evaluation and diagnosis of PM/DM-ILD.Methods: Retrospective analysis of clinical data of 38 PM/DM patients hospitalized in Tianjin general hospital and 30 heahhy subjects during May 2015-September 2017.Serum KL-6 concentrations were measured by CLIA.Compare the difference of serum KL-6 level between each group.Analysised its correlation with clinical manifestations,laboratory indexes,and HRCT manifestations.The use of the stastical package for social science(SPSS)21.0 software to analyze the difference and correlation between groups.Result: 1.The concentration levels comparison of KL-6 among PM/DM-ILD group,PM/DM-NILD group,and normal control group:The serum levels of KL-6were(1076.28±989.43),(220.92±130.31)and(99.23±35.80)U/mL,in PM/DM patients with ILD and without ILD,and healthy controls,respectively.2.Cough,cough up phlegm,weight loss,crackles and the positive rate of Ro-52 antibody,Jo-1 antibody in PM/DM-ILD group is higher than the PM/DM-NILD group,CA125,HE4 and CEA levels in PM/DM-ILD group is higher than the PM/DM-NILD group,UA levels in PM/DM-ILD below PM/DM-NILD group.3.The ROC curve showed the cut-off value of KL-6 was 402 U/ml for the diagnosis of PM/DMILD,the sensitivity,specificity,positive predictive value and negative predictive value was 68%,92.3%,94.4% and 60%,respectively.4.The correlation analysis of KL-6 levels with laboratory indexs and clinical manifestation:The serum KL-6 levels were positively related with CEA,CA153,CYFRA12-1,HE4(P<0.05);The KL-6 levels were no obvious correlation with cough,expectoration,asthmas,and so on(P>0.05).The serum KL-6 levels were positively related with myodynia and dysphagia(P < 0.05).The serum KL-6 levels were positively related with the positive rate of Ro-52 antibody,Jo-1 antibody.5.The correlation analysis of KL-6 among different HRCT groups:According to theCT appearance:mainly exudative ILD groups(grinding glass shadows);mainly fibrosis ILD groups(fiber cords).The levels of KL-6 in mainly exudative ILD groups had a significantly higher than in mainly fibrosis ILD groups(P<0.05).Conclusion:Serum KL-6 level is a potential serum marker for PM/DM patients complicated with ILD,which may contribute to early differentiate ILD from healthy people.KL-6 can be used as a predictive marker for PM/DM patients complicated with ILD.The correlation between CEA,CA153,CYFRA12-1,HE4 and PM/DM-ILD needs further study as same as the mechanism between those biomarkers and the complicating of ILD.
