Investigation And Analysis Of Unintended Pregnancy In Postpartum Women In Tianjin

Objective:With the opening of the second child policy in China,the number of re births has increased.In China,the unintended pregnancy rate of childbearing women is still at a high level,which causes the frequent occurrence of unintended pregnancy at the end of abortion,which has adverse effects on the physical and mental health of women and the harmony and stability of the family.As a period of high incidence of unintended pregnancy,how to reduce the incidence of postpartum unintended pregnancy and ensure the reproductive health of women has become a very concern for clinicians and policymakers.The purpose of this study was to investigate the situation of unwanted pregnancy in two years,including the time and causes of the recurrence of pregnancy,the outcome of pregnancy,and the time of recovery of postpartum menstruation,and through the investigation and analysis of the effect of the recovery time of postpartum menstruation,lactation and so on for the unwanted pregnancy and the situation of abortion,for postpartum guidance.To provide the basis for pregnancy,and to analyze the Contraceptive Awareness and level of postpartum women through the investigation,to provide some guidance for further propaganda of postpartum birth control education.Method :In this study,the questionnaire was used to investigate the form and content of the questionnaire,which was used by the Shanghai Institute of family planning science in 2017.A one to one questionnaire was carried out for 300 women in the obstetric department of General Hospital Affiliated to Tianjin Medical University from October 2015 to November by telephone follow up to ensure the right to privacy.The questionnaire includes the age of the women,the level of education,the occupation,the history of pregnancy,the way of delivery,the time of recovery of postpartum menstruation,the time for the recovery of postpartum sexual life,the time of breastfeeding,the first contraceptive time and the contraceptive measures.For pregnant women with repeated pregnancy,the time,reason and outcome of the second pregnancy were emphasized.Results:Among the 300 subjects,37 postpartum women were pregnant again in 2 years.Except for 4 planned pregnancies and 33 unwanted pregnancies,the rate of non willing pregnancy in 2 years postpartum was 11%.Of the 33 cases of unplanned pregnancies,8 cases had unwanted pregnancies within 1 years postpartum,so the incidence of unwanted pregnancy was 2.67% within 1 years after delivery.Among the 33 women,the age was mainly 26-35 years old,24(72.73%);the level of education was still concentrated in college / University,for 21(63.64%);the occupations now focused on white collar and family women,29(87.88%)after the first childbirth,10(30.30%)in childbirth,and 2 in cesarean delivery.3 cases(69.70%),16 of them failed in contraception,and 17 cases did not.The pregnancy outcome included 23 cases of induced abortion,1 cases of spontaneous abortion,and 9 cases of continuous pregnancy to childbirth.Among 33 postpartum women with non willing pregnancy,the median time of postpartum recovery was 3 months,and the recovery of menstruation was 60.60% in 3 months postpartum;78.78% of women recovered their sexual life in 6 months postpartum;the time for breastfeeding was more than 12 months,accounting for 66.67%.Among the 300 subjects,the first contraceptive measures were mainly condoms(71.76%).At present,263 cases(87.67%)were contraception,mainly taking contraceptive measures as condoms in 205 cases(77.95%),33 cases(12.55%),14 cases(5.32%)and 6(2.28%).90% of the puerperas began to have contraception immediately after resuming sexual life after delivery.Conclusion:1.At present,the incidence of unwanted pregnancy is still at a high level,which is significant in 2 years.Therefore,it is important to do postpartum contraceptive guidance work well.2.The lack of contraception awareness and lack of contraceptive knowledge are the main causes of postpartum unwanted pregnancy,and postpartum contraception education and postpartum contraception guidance should be strengthened.3.In the 2 years after delivery,90% of the women were more willing to use condoms and safety period to achieve the goal of postpartum contraception,but the contraceptive rate was higher.4.Unwanted pregnancy after postpartum was related to the time of recovery of postpartum menstruation,the duration of breastfeeding,the time of postpartum recovery,and the choice of contraceptive measures.Postpartum breastfeeding for a long time,late menstrual recovery time and post natal choice of effective contraceptive measures are conducive to reducing the incidence of postpartum unwanted pregnancy.
