ObjectiveThe clinical benefits of carbohydrate intake 2-3 hours before surgery in patients with gastrointestinal cancer without diabetes mellitus have been confirmed by foreign experts,literature and guidelines with “High level evidence” for perioperative nutrition support are also written in china Guidelines for perioperative nutritional support in adults.However there are few hospitals in china that prefer preoperative glucose pretreatment,taking into account the differences between domestic and foreign data.In this paper,we analyzed the clinical randomized controlled experimental data about the effect of preoperative glucose pretreatment on the prognosis of patients with malignant tumor of digestive tract by Meta analysis,and to evaluate the efficacy and safety of preoperative glucose pretreatment in Chinese patients,and to achieve the clinical significance.MethodsThe related literature retrieval sugar pretreatment and prognosis of malignant tumor of digestive tract surgery was searched In database and the data taken were before 2017-03.Of the total search 10 randomized controlled trials met the inclusion criteria by Revman5.3 software(606 patients)were Meta analyzed.ResultsCompared with the traditional fasting and non drinking group,the preoperative group was significantly reduced(SMD=-1.29,95%CI=-2.00~-0.59,P=0.0003),Postoperative insulin resistance(Homa-IR)was significantly reduced(SMD=-1.92,95%CI=-2.68~-1.16,P<0.01),postoperative blood glucose concentration,insulin concentration increased,and no vomiting and aspiration occur in any patient as shown by the literature.ConclusionIn the patients with malignant tumor of digestive tract perioperative nutrition support and pretreatment shows significant clinical benefit,preoperative subjective comfort,improve the operation risk of aspiration significantly and postoperative insulin resistance.Clinical benefit was confirmed with preoperative glucose pretreatment in the domestic application and clinical benefit for the digestive tract malignant tumor surgery,which is Safe and effective.
