Objective:To investigate the relationship between the level of uric acid in patients with leukoaraiosis and the degree of leukoaraiosis,and explore the effects of serum uric acid on cognitive impairment.Methods:According to the inclusion and exclusion criteria,222 patients with leukoaraiosis who were admitted to the Affiliated Hospital of Chengde Medical University from April 2016 to March 2017 were collected.The patient's general epidemiological data and laboratory test indicators were collected.The Chinese version of the Montreal Cognitive Assessment Table was used to score patients' cognitive function.Using the Fezakas scale for leukoaraiosis:1-2 were divided into mild leukoaraiosis group(n=124),3-6 were divided into moderate-severe leukoaraiosis group(n=98);According to uric acid levels,they were divided into high uric acid group(male?416?mol/L,female?358?mol/L)and low uric acid group(male<416?mol/L,female<358?mol/L).The biochemical indicators such as uric acid of the study subjects were separately tested.The general epidemiological data,biochemical indicators,and cognitive functions were compared between groups.Spearman correlation analysis was used to analyze the correlation between serum uric acid levels and leukoaraiosis.Logistic regression was used to analyze the influencing factors of cognitive impairment.Results:1 Comparison of clinical data between two groups of LA patientsThe serum uric acid in the moderate-severe LA group was(324.62±87.49)?mol/L,which was significantly higher than that in the mild LA group(297.76±79.62)?mol/L.The difference in uric acid levels between the two groups was statistically significant(t=-2.39,P=0.02);The MoCA score in the moderate-severe LA group was(20.64±3.76)points,which was significantly reduced than that in the mild LA group(23.70±4.70).There was a statistically significant difference in the MoCA score between the two groups(t= 5.25 P<0.01).There was no statistical difference in other data(P>0.05).2 Comparison of clinical data between high UA group and low UA group in patients with mild leukoaraiosisThe MoCA score in the high UA group group was(21.44±4.43)points,which was significantly lower than that of the low UA group(25.78±5.01).The difference was statistically significant(t=-5.18,P<0.01).There was no statistical difference in other data(P>0.05).3 Comparison of clinical data between high UA group and low UA group in patients with moderate-severe leukoaraiosisThe MoCA score in the high UA group group was(17.46±3.69)points,which was significantly lower than that of the low UA group(22.08±3.85).The difference was statistically significant(t=-6.04,P<0.01).There was no statistical difference in other data(P>0.05).4 Spearman correlation analysis was used to analyze the correlation between serum uric acid and leukoaraiosis.The results showed that serum uric acid levels were positively correlated with the Fezakas score(r=0.198,P?0.01).Serum uric acid levels were negatively correlated with the MoCA score(r=-0.21,P<0.01).5 With uric acid,Fezakas score as independent variables,the degree of cognitive dysfunction is a dependent variable.After multivariate logistic regression analysis,the results showed that uric acid(OR=1.01,95% CI 1.00-1.01,P=0.01)and leukoaraiosis(OR=4.18,95% CI 2.90-6.02,P<0.01)were the independent risk factors for cognitive impairment.Conclusion:1 The level of uric acid in patients with leukoaraiosis is positively correlated with the degree of leukoaraiosis.The higher the level of uric acid,the more severe the white matter quality of the brain.2 The level of uric acid in patients with leukoaraiosis is positively correlated with cognitive impairment.Uric acid in patients with leukoaraiosis are independent risk factors for cognitive dysfunction.
