Objective:To investigate the diagnostic value and complications of CT-guided percutaneous needle biopsy of peripheral lung cancer.Methods:A retrospective analysis of 144 patients with peripheral lung lesions with CT guided puncture biopsy in our hospital from 2015-1 to 2017-5 was retrospectively analyzed.Statistical puncture pathology type;The accuracy,sensitivity,false negative rate and false positive rate were calculated.To calculate the total pneumothorax,the incidence of hemorrhage and the analysis of different lesion size,lesion location and the length of the lesion on the periphery of the lesion.Results:In all 144 cases,the patients were successfully punctured successfully.Among them,there were 119 cases of malignant malignancy,25 cases of benign or necrotic tissue.Finally,there were 124 cases of malignant patients undergoing surgery,pathology,clinical treatment or follow-up diagnosis,and 20 cases were benign.The rate of puncture biopsy was 95.83%,the sensitivity was 95.20%,the specificity was 100%,the false negative rate was 4.80%,and the false positive rate was 0%.The incidence of pneumothorax was(21/75)28.00%,and the incidence of bleeding(18/75)24% was significantly higher than that in the > 3cm group(7/69)10.14%,(6/69)8.70%,and the difference was statistically significant(x2=7.314,P < 0.05.x2 =6.061,P < 0.05).Needle after lung distance of 2 cm or less incidence of pneumothorax was 10.84%(9/83),the incidence of bleeding was(8/83)9.64%,who are below after lung distance is greater than 2 cm group 31.00%(19/61),26.23%(16/61),the differences having statistical significance(x2 = 9.254,P 0.05 or less;x2 =6.968,P < 0.05).The gender,age and lesion position of the patient had no significant effect on the incidence of pneumothorax and hemorrhage(P > 0.05).Conclusion:Percutaneous puncture biopsy under the guidance of CT is highly accurate and safe for peripheral pulmonary lesions.The reasonable mastery of puncture technique can improve the diagnostic efficiency and can be used as an effective qualitative method for diagnosis of peripheral lung cancer.
