Diabetic foot(DF)is one of the serious chronic complications in diabetic patients,which is related to vascular and nerve,injury,infection and other f actors caused by abnormal metabolism.Infection further exacerbates the severit y of foot ulcers.However,there have been extensive changes in clinical treat ment and environment in recent years,the bacterial characteristics of DF infec tion vary greatly,mixed infection of various bacteria increases with the exac erbation of DF,and prone to multidrug resistance,the difficulty of treatment increased.Therefore,it is important to understand the characteristics and influe ncing factors of DF mixed bacterial infection for improving the prognosis of DF patients.Objective:To study diabetic foot mixed bacterial infection characteristics and influencing factors,reduced the incidence of mixed bacterial infection in DF patients and improved the prognosis of patients.Methods:1.114 cases of diabetic ulcer with complete case data treated by our hospital from June 2010 to March 2017 were collected,Age,sex,wagner grade,foot ulcerative disease course,history of antimicrobial drugs,results of first auxiliary examination after admission including bacterial culture and drug sensitivity test results,fasting blood-glucose,glycosylated hemoglobin,plasma albumin,triglyceride,total cholesterol,low-density lipoprotein,high-density lipoprotein,ankle-brachial index(ABI),transcutaneous oxygen tension(TcPO2)were analyzed.the result of bacterial culture was observed,the number of mixed bacterial infections,the characteristics of bacterial infection were analyzed.2.Comparison of the distribution of mixed bacterial infections in differentgrades were analyzed by chi-square test,variables having statistical significance by single factor analysis were further analysed by Logistic stepwise regression analysis for influence factors of mixed bacterial infection in diabetic foot patients.P<0.05 was considered statistically significant.Results:1.There were 105 positive bacterial cultures in this group,59 cases single bacterial infection(56.2%),46 mixed bacterial infections(43.8%).and the bacterial infection rate increases with the severity of the disease(P<0.05).2.Mixed bacterial infections were mainly gram-postive bacteria and gram-negative bacteria,gram-negative bactera and gram-negative bacteria.And that 20 cases of multidrug-resistant bacteria were found in mixed bacterial infection,the infection rate was 43.5%,which was significantly higher than that of patients with single bacterial infection.32 strains of multidrug-resistant bacteria were developed in mixed bacterial infection,the top three were Staphylococcus aureus,Proteus and Pseudomonas aeruginosa.3.Logistic stepwise regression analysis showed that long diabetic foot course,having a history of antibiotics,high Wagner grade,low ABI and low(transcutaneous oxygen pressure)TcPO2 were the influence factors of mixed bacterial infection.Conclusion: Understanding of bacterial characteristics and associated influencing factors in mixed bacterial infections,Strengthening anti infection and prevention measures,to reduce the infection rate of DF mixed bacteria and multidrug-resistant bacteria,and to improve the prognosis of diabetic foot patients.
