| Objective: Type 2 diabetes mellitus(T2DM)main characteristic is abnormal glucose metabolism,patients often have overweight/obesity 、dyslipidemia、hypertension and other diseases,these can endanger patients’ health seriously.Metabolic syndrome(MS)is a clinical syndrome,which is characterized by abnormal aggregation of metabolic components,such as glucose,blood pressure,blood lipids,body mass index.At present,T2 DM and MS have become a more serious public health problem,many researchers pay more attention to it.This retrospective case-control study aims to analyze the clinical characteristics of T2 DM patients,explore the relationship between T2 DM and MS components,risk factors associated with MS,finally provide a theoretical basis for clinical intervention in the occurrence and progression of T2 DM and MS.Methods: 3251 hospitaled T2 DM patients from January 2014 to May 2016 in the endocrinology department of Baoding First Central Hospital were collected,recorde patients’ age,sex,height,weight,DM course,DM family history,smoking history(5 consecutive years,an average of 10 and more per day),Measure blood pressure(BP),fasting plasma glucose(FPG),Hemoglobin Alc(Hb A1c),total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),blood urea nitrogen(BUN),creatinine(CR),serum uric acid(SUA),alanine aminotransferase(ALT)and Aspartate transaminase(AST),γ-glutamyl transpetidase(γ-GT)and fibrinogen(FIB),calculate the number of vascular complications in T2 DM patients.Analyse clinical information between different groups according to patients’ gender,whether apper MS,thenumber of MS components.SPSS 19.0 software was used for statistical analysis,normal distribution measurement data were expressed by sx ±,comparison between the two groups were expressed by independent samples t test,multiple groups were expressed by one-way ANOVA,enumeration data were expressed by rate(%)and cases,Comparison between groups were uesed χ2 test,logistic regression were used to analyze the related risk factors of metabolic syndrome components in type 2 diabetes mellitus patients.The difference was statistically significant when P<0.05.Results: 1 BMI,DBP,TG,ALT,γ-GT,BUN,CR,SUA and macrovascular complications in male T2 DM patients were significantly higher than female T2 DM patients(P <0.05);the age,DM course,SBP,TC,HDL-C,LDL-C and FIB in male T2 DM patients were significantly lower than female T2 DM patients(P <0.05);AST,FPG,Hb A1 c and the number of microvascular complications in male T2 DM patients and female T2 DM patients were not statistically significant(P> 0.05).2 The prevalence rate of MS in T2 DM patients was 69.15%,the prevalence rate of MS in male T2 DM patients was 70.13%,the prevalence rate of MS in female T2 DM patients was 68.01%,there was no significant difference in the prevalence rate of MS in T2 DM patients with different sex(P> 0.05);the prevalence rate of dyslipidemia and overweight/obesity in male T2 DM patients was higher than female T2 DM patients,the prevalence rate of hypertension was lower than female T2 DM patients(P <0.05).3 The age,disease course,BMI,SBP,DBP,TC,TG,LDL-C,BUN,SUA and the number of vascular complications in MS group patients were significantly higher than non-MS group patients,HDL-C and Hb A1 c were significantly lower than non-MS group patients(P <0.05),ALT,AST,γ-GT,CR,FPG,FIB between two groups showed no significant difference(P> 0.05).4 The age、disease course,BMI,SBP,DBP,TC,TG,HDL-C,LDL-C,ALT,AST,BUN,SUA,the number of vascular complications increased from MS1 to MS4 group,but HDL-C,Hb Alc showed a downward trend,the differences were statistically significant(P <0.05).5 Logistic regression analysis showed that male and HDL-C were the protective factors of MS in T2 DM patients;the age,long-term smoking history,BMI,SBP,DBP,TG and the number of macrovascular complications were the risk factors of MS in T2 DM patients(P <0.01).Conclusions: 1 Male T2 DM patients are more likely to appear dyslipidemia,overweight/obese,and female T2 DM patients are more likely to appear hypertension.2 With the abnormal number of MS components increased,metabolic disorder showed a serious trend.3 The independent risk factors of MS in T2 DM patients are: the age,long-term smoking history,BMI,SBP,DBP,TG,and the number of macrovascular complications;recommending early screening for high-risk groups can prevent T2 DM and MS from happening and developing to some extent. |
