Purpose:Through an analysis of the clinical data of 82 patients with ? and ? stage NSCLC.Analyzing the relationship between clinical data and survival time of these patients and exploring the factors which affecting the prognosis of patients with ? and? stage NSCLC,to provide the basis for patients prognosis judgement.Methods:A study was used to collect the medical records of ? and ? stage NSCLC patients who were hospitalized in Affiliated Hospital of Yan'an University during the period of January-2017 January 2014.And summarized 82 patients who met the criteria of inclusion and exclusion in this study.The patients' age,sex,PS score,clinical stage,pathological type,smoking index,Pre treatment tumor marker and coagulation indicator were recorded,and follow up the survival time of these patients.SPSS software 20.0 was used for univariate analysis of all research indicators.Then,putting the statistically significant predictors which were clearly selected by univariate survival analysis into the Cox regression model for multivariate survival analysis.All patients were stratified after analysis according to the Chemotherapy cycle,and then the indicators of prognosis before treatment were screened out.Finally,the relationship between the combined predictors and the overall survival time was analyzed.Results:1.A total of 82 patients with ? and ? stage NSCLC were included,of which 63 were dead,19 were censored,and the rate of consored statas was 23.2%.The total median survival time of the patients was 14 months.2.Univariate survival analysis showed that there was no significant difference between gender,smoking index,pathological type,NSE,Cyfra21-1,CA125 and survival time(P > 0.05);age,clinical stage,PS score,Chemotherapy cycle,CEA and D-Dimerwere related to the survival time of ? and ? stage NSCLC(all P < 0.05).3.Multivariate survival analysis clearly showed that clinical stage,PS score,chemotherapy cycle and CEA was the independent prognostic impact factors of NSCLC patients with ? and ? stage.The risk ratios(HR)were 2.460,4.564,3.377,3.109,respectively.4.All 82 patients were stratified according to the chemotherapy cycle,and the results showed that in the chemotherapy cycle <4 group,only PS score was the independent risk factors(HR=5.953,P=0.005)and in more than 4 cycle chemotherapy group,clinical stage,PS score and CEA is the independent risk factors of prognosis for patients,The risk ratio(HR)were 2.555,3.333,3.163,respectively.5.The independent risk factors of prognosis in patients with more than 4chemotherapy cycle were divided into 0,1,2,3 group,according to the number of the number of independent risk factors.The results showed that the median survival time(MST)of patients without any prognostic independent risk factors was 25 months,the MST of 1 group was 18 months,the MST of 2 group was 14 months,the MST of 3group was 11 months.Conclusion:1.Age,clinical stage,PS score,chemotherapy cycle,serum CEA and D-Dimer rise before treatment were related to the prognosis of ? and ? stage NSCLC.Clinical stage,PS score,chemotherapy cycle and serum CEA rise were independent predictors of the prognosis of ? and ? stage NSCLC.2.For more than 4 cycles of chemotherapy patients,clinical stage,PS score and serum CEA rise before treatment were independent predictors of prognosis and chemotherapy <4 cycle group only PS score has independent predictive value.3.In more than 4 chemotherapy cycle group,the more independent predictors,the shorter the survival time,the worse the prognosis of patients.
