Objective Acute ischemic stroke is a common and frequently disease of the nervous system,the mortality rate of all diseases accounted for 6%,the third cause of death of mankind,statistics show that about 50% to 70% survivors left paralysis,aphasia Serious disability,serious harm to human health and quality of life,to the community and the family a heavy burden.Due to acute ischemic stroke mortality and disability,combined with the death of nerve cells after the non-renewable,the current international investment in a large number of funds committed to post-stroke treatment research.Since the publication of the NINDS study in 1996,alteplase has become an effective means of reperfusion in acute ischemic stroke.NINDS study to head CT as one of the criteria for judging,but the head CT for the potential risk of intracranial hemorrhage can not be a small lesion and brain MRI as clear discrimination,it has some limitations.The aim of this study was to investigate the safety of alteplase thrombolysis in patients with acute ischemic stroke complicated with microbleeds,and to be able to guide clinical trials.Methods In this study,74 cases of middle cerebral infarction with cerebral microbleeds were selected from May 2015 to May2017.The patients in the control group were treated with antiplatelet therapy,37 patients in the observation group and intravenous thrombolytic therapy After treatment in the efficacy,complications,m RS score,NHISS score changes.Results The curative effect was 18.92%,the rate of deterioration was 13.51% and the total effective rate was 72.98%.The cure rate was 32.43%,the rate of deterioration was 2.7% and the total effective rate was 86.49%.The difference between the two groups was significant(P <0.05).At 2h,24 h,7d and 14 d after treatment,there were significant differences between the two groups before and after treatment(p <0.05).After treatment,the observation group improved 2h,24 h and 7d after treatment(P<0.05).The incidence of intracranial hemorrhage and vascular occlusion in the control group were 0,16.22% and 24.32%,respectively.The incidence of intracranial hemorrhage and vascular occlusion in the observation group were significantly higher than those in the control group(p <0.05)(P <0.05).The incidence of m RS score was 48.65% and the incidence of 2-6points in the control group was 51.35%,and the observation group was 0-1.5%,5.41% and 5.41% The incidence of 1 points was72.97%,and the incidence of 2-6 was 27.03%.There was significant difference between the two groups(p <0.05).Conclusion Intravenous thrombolysis of alteplase is safe and effective in acute ischemic stroke with cerebral microbleeds.
