Objective: 1?Evaluate the level of symptom distress and the level of hope in colorectal cancer patients with permanent enterostomy.2?Analyze the correlation between the level of symptom distress and the level of hope in colorectal cancer patients with permanent enterostomy.3? Built hope therapy based on Snyder's hope theory,establish reasonable intervention mode.4? Explore the application effect of hope therapy in colorectal cancer patients with permanent enterostomy.Methods: Based on the random sampling method to select a 3 armour hospital in Shanghai in October 2016 to December 2017 in gastrointestinal surgery for colorectal cancer permanent intestinal colostomy surgery and meet eligibility criteria of the 108 patients,randomly divided into intervention group and control group ? The intervention group included 55 cases,The control group included 53 cases.The control group was given routine care.In addition to routine care,the intervention group implemented comprehensive nursing intervention on the basis of hope therapy,including in-hospital intervention and out-of-hospital follow-up.The whole intervention period was 3 months after the postoperative week.With Anderson symptom survey scale,Hector hope scale,colostomy complications record score as the main evaluation index,evaluation of patients with postoperative week,1 month and 3 months after hospital discharge level and symptom distress and hope 1 month and 3 months after hospital discharge colostomy complications.Collected and sorted the data into the database,and used spssl6.0 to analysis of the data,so as to explore the intervention effect of hope therapy.Results: 1?The total score of Symptom distress of one week after surgery was(3.35±4.86),and 83.3% of the patients were mild.Among them,the first five items of each item were not fatigue,dry mouth,sadness,pain and insomnia.The first three of the items in the list of symptoms that interfere with life are emotions,general activities,and relationships with others.2?The total score of hope level of one week after surgery was(34.30 ±3.08),and 94.1% of patients were in the middle and higher levels.3?There was a significant negative correlation between the total score of symptoms distress and the level of hope and its dimensions,(P < 0.05).4?Before the intervention,the score of symptoms distress of the control group was 3.31±3.44 and the intervention group was 3.41 ±2.52,P=0.825,the difference was not statistically significant(P>0.05).but After 3 months of intervention,the score of the control group was 2.72±6.23 and the intervention group was 1.26 ±7.50,P=0.001,the difference was statistically significant(P<0.05).5?Before intervention,the score of the level of hope of the control group was 35.76±8.28 and the intervention group was 34.32±3.98,P=0.208,the difference was not statistically significant(P>0.05).but After 3 months of intervention,the score of the control group was 36.43±5.42 and the intervention group was 42.37±8.51,P < 0.000,the difference was statistically significant(P<0.05).6?Produce situation of colostomy complications between intervention group and control group was no significant difference on the day of discharge(P > 0.05);two groups of patients with colostomy surrounding skin,colostomy stenosis,colostomy complications surrounding skin mucous membrane separation and serious level have obvious difference 3 months after discharge(P < 0.05);However,there was no statistically significant difference in the observed time between the prolapse of the mouth,the parastomal hernia,and the contraction of the mouth.(P>0.05).Conclusion: 1?The symptoms disterss of colorectal cancer patients with permanent enterostomy were mild.2?The level of hope for colorectal cancer patients with permanent enterostomy was generally mild and moderate.3?The higher the symptoms distress of colorectal cancer patients with permanent enterostomy,the lower the level of hope.4?Hope therapy can reduce the symptom distress level and postoperative complications of colorectal cancer patients with permanent enterostomy and improve their level of hope.
