Objective:Design a novel transoral atlantoaxial dislocation reduction device,and it can not only assist in the implantation of self-implantation of autogenous tibial and/or transoral mass fusion cage of current transoral atlantoaxial reduction plate(TARP),but also can assist in the implantation of the fusion cage in the zero-segment atlanto-axial lateral interbody fusion fixation system at the same time of satisfied reduction of atlantoaxial.And it can make the reduction of irreducible atlantoaxial dislocation safer,more accurate and satisfied.Method:15 axial dry bone specimen and the atlantoaxial thin-layer CT 3D reconstruction Data of 40 normal adultswere collected for anatomical and radiographic measurements respectively.The data of the commonly used Codman hooks for oral surgery were measured.The atlantoaxial thin-slice CT three-dimensional reconstruction data of 40 patients with irreducible atlantoaxial dislocation who underwent the surgery of transoral atlantoaxial reduction plate(TARP)were used to measure the ideal atlantoaxial reductionn distance.Refer to the relevant data of the measurement to design a novel transoral atlantoaxial dislocation reduction device Results:1.Anatomic measurements of the mean diameter of the minimum transverse diameter(L1)of the odontoid of the axis is 8.44±1.00mm;the mean value of the maximum sagittal diameter(L2)of the axial vertebral body is 16.64±1.60mm;Imaging measurements of the mean diameter of the minimum transverse diameter(D1)was 8.45±0.96 mm;the mean value of the maximum sagittal diameter(D2)of the axial vertebral body was 17.30±1.67 mm,and there was no significant difference between the anatomical measurement data and the imaging measurement data(P >0.05)2.The mean value of ideal atlantoaxial reductionn distance before and after irreversible atlantoaxial dislocation is 8.56±2.52 mm.The average value of the ideal updown reduction distance is 7.30±1.91 mm.There is no statistical difference between the left and right sides.3.Design a new type of transoral atlantoaxial dislocation reduction device with the measurement and statistical data.Conclusion:The new type of transoral atlantoaxial dislocation reduction device which can quantify the degree of reduction of the atlantoaxial and reduce the incidence of related complications is novel in design and has the advantages of simple operation,accurate and safe reduction,etc.It can be promoted and applied in clinical practice combined with transoral atlantoaxial reduction plate TARP surgery and zero-track atlanto-axial lateral interbody fusion fixation system.
