| Objective 1.To establish the perioperative nursing standard in internal fixation of transoral atlantoaxial reduction plate for clinical application.2.The perioperative nursing standard in internal fixation of transoral atlantoaxial reduction plate was applied to clinical practice,to reduce postoperative complications,improve the quality of nursing and the effect of surgical treatment.3.To avoid the phenomenon of medical nursing disconnection during the popularization of internal fixation of transoral atlantoaxial reduction plate,to realize the synchronous development of medical nursing discipline,promote its application and development,benefit more patients in need.Methods 1.Through the literature review,to study the present situation and the latest progress in perioperative nursing research of internal fixation of transoral atlantoaxial reduction plate,and to formulate the first draft of perioperative nursing standard in internal fixation of transoral atlantoaxial reduction plate.2.Constituted an expert review team,revised it repeatedly,and finally formed the perioperative nursing standard in internal fixation of transoral atlantoaxial reduction plate.3.The perioperative nursing standard in internal fixation of transoral atlantoaxial reduction plate was applied to clinical,94 patients who were admitted to a tertiary hospital in Guangzhou were randomly divided into control group and experimental group.The perioperative routine nursing and the perioperative standard nursing were used respectively.Collected evaluation indicators to evaluate clinical effect,including the perioperative anxiety score,the gastric tube indwelling time,the tracheal catheter indwelling time,the postoperative complication rate,the hospitalization time,the hospitalization expenses and the satisfaction of patients.The self-designed questionnaire was used to investigate the satisfaction of patients.The statistical analysis was carried out with the SPSS17.0.T-test,chi-sqare test and rank sum test were used in process of analyzing.The were a statistically difference at the 0.05(2-tailed).Results 1.Form the perioperative nursing standard in internal fixation of transoral atlantoaxial reduction plate,including three clinical nursing stages,twenty-three nursing measures,five perioperative related forms,one clinical nursing pathway and an oral nursing procedure.2.Clinical application effect of the perioperative nursing standard in internal fixation of transoral atlantoaxial reduction plate 2.1 The perioperative anxiety score: On admission,the control group anxiety score was(59.420±8.440),the experimental group was(58.370±8.660).There was no significant difference between the two groups(P>0.05).Before surgery,the control group anxiety score was(62.920±3.463),the experimental group was(49.410±7.219).The experimental group preoperative anxiety was lower than the control group,the comparison between the two groups was statistically significant(P<0.05).2.2 The indwelling time of gastric tube: The average gastric tube indwelling time was(11.420±7.293)d in the control group,and(7.760±1.479)d in the experimental group.The indwelling time of gastric tube in the experimental group was shortened,the comparison between the two groups was statistically significant(P<0.05).2.3 The indwelling time of tracheal catheter: The average indwelling time of tracheal catheter was(2.540±0.922)d in the control group,and(1.700±0.662)d in the experimental group.The average indwelling time of tracheal catheter in the experimental group was shortened,the comparison between the two groups was statistically significant(P<0.05).2.4 The Incidence of postoperative complications: There were 35(72.92%)cases of postoperative complications in the control group,including 10(20.83%)cases of pulmonary infections,20(41.67%)cases of hyponatremia,3(6.25%)cases of incision infection and 2 cases of cerebrospinal fluid leakage(4.17%);13(28.26%)cases of postoperative complications in the experimental group,including 3(6.52%)cases of pulmonary infection,7(15.22%)cases of hyponatremia,2(4.35%)cases of incision infection and 1(2.17%)cases of cerebrospinal fluid leakage.The incidence of postoperative complications in the experimental group was lower than in the control group,the comparison between the two groups was statistically significant(P<0.05).2.5 The hospitalization time: The average hospitalization time of the control group was 17d(14.00,22.00),15d(12.00,23.00)in the experimental group.The average hospitalization time in the experimental group was shortened,the comparison between the two groups was statistically significant(P<0.05).2.6 The hospitalization expenses: The average hospitalization expenses was(11.140±11.374)ten thousand yuan in the control group,(7.478±1.172)ten thousand yuan in the experimental group.The hospitalization expenses in the experimental group was lower than in the control group,the comparison between the two groups was statistically significant(P<0.05).2.7 The satisfaction of patients: The satisfaction of was(92.460±4.267)in the control group,(95.350±6.416)in the experimental group.The satisfaction of patients in the experimental group was higher than in the control group,the comparison between the two groups was statistically significant(P<0.05).Conclusion This study showed that using the methods of literature review and expert review to establish the perioperative nursing standard in internal fixation of transoral atlantoaxial reduction plate could reduce the perioperative anxiety level of patients,shorten the indwelling time of gastric tube and tracheal catheter in patients,reduce incidence of postoperative complications,shorten hospitalization time,reduce hospitalization expenses and improve nursing satisfaction.It had good clinical practicability and was available for clinical application and reference. |
