Objective:To explore the relationship between drug abusing and unprotected anal intercourse(UAI)among men who have sex with men(MSM),to compare Acquired Immunodeficiency Syndrome(AIDS)related characteristics between abusers and non-abusers,and to understand the relationship between drug abusing and sexual transmitted infection.Methods:A cross-sectional study was conducted.Abusers and non-abusers were recruited in a snowball way.Face-to-face interviews were conducted to collect their demographic information,drug abusing,sexual behaviors characteristics,etc.Respondents filled in Social Support Rating Scale with the help of investigators.Blood samples were collected to test Human Immunodeficiency Virus(HIV),syphilis and Hepatitis C Virus(HCV)antibodies.Urine samples were collected to test methamphetamine,ketamine,etc.Qualitative variables were compared using Chi-square test,and quantitative variables were compared using Student's t-test or Wilcoxon signed-rank test.Univariate and multivariate logistic regression was used to analyze the relationship between drug abusing and UAI,a=0.05.Results:1.AIDS related characteristics of abusers and non-abusers.A total of 602 MSM were recruited,including 302(50.2%)abusers and 300(49.8%)non-abusers.The average age of all respondents was 27.9±6.7.Compared with non-abusers,abusers were more likely to be unmarried,to have registration of external city,to have been living in Qingdao less than 5 years,P<0.05.During the last 6 months,the proportions of using condoms continuously with regular male partners(RMP)among abusers and non-abusers were 13.0%and 33.1%respectively,P<0.01,the proportion of condom breakage during sexual behaviors with RMP in abusers(48.2%)was higher than that in non-abusers(26.6%),P<0.01.During the last 6 months,the proportion of condom breakage during sexual behaviors with non-regular male partners(NRMP)in abusers(39.7%)was higher than that in non-abusers(27.6%),P<0.05.The proportions of having commercial male partners(CMP)among abusers and non-abusers were 22.3%and 10.7%respectively,P<0.01.46.0%of all respondents had group sex during the last 6 months,the proportions were 68.5%and 23.3%among two groups,P<0.01.The total prevalence of HIV,syphilis and HCV infection were 0.7%,4.3%and 0.0%,respectively.There was no statistically significant difference in HIV infection rate between abusers and non-abusers.The prevalence rate of syphilis infection in abusers(2.6%)was lower than that in non-abusers(6.0%),P<0.05.2.The influencing factors of UAI.According to multivariate logistic regression,having insertive/receptive sexual preference(OR=2.35,95%CI:1.37-4.04),having>2 types of male sexual partnters.(OR=1.82,95%CI:1.14?2.91),getting a social support scale score greater or equal to 32(OR=2.48,95%CI:1.47-4.20)and abusing drug(OR=1.77,95%CI:1.06-2.96)were risk factors of UAI during the last 6 months.3.The relationship between drug abusing and UAI.59.6%of all abusers used methamphetamine,56.0%abused 5-Methoxy-N,N-diisopropyltryptamine(5-MeO-DIPT,"foxy"),and 32.5%abused Rush Poppers.Subgroup analysis showed that abusing drugs in bars was a risk factor of UAI during the last 6 months,OR=2.34(95%CI:1.01?5.43).4.The relationship between drug abusing and syphilis infection.MSM with receptive sexual preference were more likely to infect syphilis,OR=2.83(95%CI:1.02?7.86).There was no statistically significant correlation between drug abusing and syphilis infection,P=0.067.Conclusion:Drug abusing is a risk factor of UAI during the last 6 months.Compared with non-abusers,drug abusing MSM had higher prevelence of UAI with regular male partners and group sex.Meanwhile,abusers were vulnerable to condom breakage and HIV infection.HIV and syphilis testing,surveillance on high-risk behaviors and health education on condom using should be strengthened for drug abusers.
