Study On Clinical Characteristics And TCM Syndromes Of 6095 Inpatients With AIDS

Objective:Since its discovery,AIDS has spread rapidly and has become a public health and social issue that seriously affects human health and social development.At present,the treatment of AIDS is mainly Highly active antiretroviral therapy(HAART),ie,“cocktail therapy”.Although it can effectively reduce the viral load of patients and improve clinical symptoms,there are also many problems,such as serious side effects,drug resistance,and poor patient compliance.Chinese medicine has certain advantages in the treatment of AIDS.However,due to the specificity of this disease,the patient population is hidden.Therefore,clinical research on AIDS is mostly epidemiological investigation or case-control study of small samples,lack of large-sample systematic research.Based on this,this study used a retrospective study method to collect,count,and analyze information about the history of AIDS patients admitted to hospitals in Shangcai County,Henan Province from 2011 to 2017 to explore the clinical features and TCM syndrome characteristics of these patients.Establish a database to lay the foundation for follow-up research.Methods:Extract the medical records of AIDS patients in Shangcai County Hospital of Henan Province from 2011.01 to 2017.11.The information engineers in the Information Department of the First Affiliated Hospital of Henan University of Traditional Chinese Medicine(CAU)helped to extract,classify,and summarize the information on a certain fixed module in the electronic medical records,such as engraved disease,past history,and diagnosis of traditional Chinese medicine,and establish a database.Through the cleaning of data,code conversion,establish a standardized,statistical database.Descriptive statistical analysis was performed using SPSS19.0 and Excel 2016.Results:1 Basic information of 6095 inpatients with AIDSA total of 6,095 cases of AIDS patients were taken from Shangcai Hospital of Traditional Chinese Medicine from 2011.01 to 2017.11.Due to the incompleteness of some electronic medical records,the total number of results of each content extraction is?6095 cases.The gender distribution of AIDS inpatients was:male 2946 cases,female 3128 cases,male to female ratio was 1:1.062.The youngest is 5 years old,the oldest is 86 years old,the average age is 50.60±7.6,and the age is mainly concentrated in the 45-65 years old(68.4%).The ethnic group is mainly Han.The marital status is mainly married.2 Clinical features of 6095 inpatients with AIDSIn the medical records of 6095 cases of AIDS patients,the main route of infection was blood donation,followed by blood transfusion and sexual transmission.Antiviral drug use:4124 cases(70.9%)adhered to antiviral drugs,1625 cases(27.9%)never took antiviral drugs or stopped due to drug side effects.58.3%of patients applied second-line antiviral therapy.The number of CD4~+T-lymphocyte counts in AIDS hospitalized patients was mainly below 200/?L,followed by 201 to 350/?L.3 Characteristics of TCM Syndromes of 6095 inpatients with AIDSThe main symptoms of 6095 AIDS patients in the top ten according to the frequency were as follows:fatigue,poor appetite,dizziness,heavy limbs,palpitations,flu,cough,insomnia,shortness of breath,headache.The main signs are superficial lymphadenopathy,skin lesions(macula,pimples,herpes,etc.),oral leukoplakia,ulcers,etc.Comorbidities or opportunistic infections are hypertension,hepatitis,coronary heart disease,cerebral infarction,diabetes,tuberculosis,Pneumocystis carinii pneumonia(PCP),pulmonary infections,and cerebral hemorrhage.The most frequently occurring tongue color in the 6095 cases of AIDS patients was pink tongue,followed by red tongue and light white tongue.The most common type of moss is thin moss,followed by greasy moss and thick moss.The most common color of moss is white moss,followed by yellow moss.The thin pulse frequency is the highest,followed by sunken pulse and wiry pulse.The highest frequency of TCM syndromes is qi,blood,yin and yang deficiency syndrome,followed by qi and blood deficiency syndrome,blood stasis syndrome,and phlegm dampness syndrome.The property of pathological change is mainly based on deficiency syndrome,and the disease position is mainly responsible for the spleen,accumulating lung,kidney and liver.4 Correlation between CD4~+T-lymphocyte Count and TCM Syndrome Types in AIDS InpatientsWhen CD4~+T lymphocyte count is less than 200/?L,syndrome type is mainly deficiency syndrome.When the number of CD4~+T lymphocytes was 201-350/?L,the syndrome distribution was 35.0%of deficiency syndrome,35.0%of excess syndrome,and 30.0%of deficiency-excess complex syndrome.When CD4~+T lymphocyte count is more than 350/?L,the syndromes were mainly excess syndrome.The results of chi-square test,P<0.05,the difference between the groups was statistically significant.Conclusion:The main infection route of AIDS patients in Shangcai County Hospital of Henan Province is paid blood donations.The most common opportunistic infection is lung infection.AIDS patients often co-infected with viral hepatitis C.The application rate of antiviral drugs of Shangcai Hospital of Traditional Chinese Medicine is 70.9%,and the second-line antiviral treatment program for AIDS patients is the most widely used.The main symptoms of AIDS patients are mainly deficiency symptoms,the main evil is wet evil.Common signs include superficial lymphadenopathy,oral leukoplakia,oral ulcers,skin papules,and herpes.The syndrome is mainly based on deficiency syndromes,mainly qi and blood,yin and yang deficiency,qi and blood deficiency,and spleen deficiency.The disease location is mainly spleen and lung.There is a certain correlation between CD4~+T-lymphocyte counts and TCM syndrome types in AIDS patients.When the count of CD4~+T lymphocytes is less than200/?L,the syndrome type is mainly based on deficiency syndromes.When the CD4~+T lymphocyte count is more than 350/?L,the syndrome is mainly based on excess syndrome.In the course of a gradual decrease in the count of CD4~+T lymphocytes,the syndromes evolved from excess syndrome to deficiency-excess complex syndrome,and deficiency syndromes.CD4~+T lymphocyte count has certain reference significance for the determination of TCM syndrome type.
