Objective:The aim of this research is to study the relationship between condylar morphology,condylar position and temporomandibular disease by using CBCT,furthermore,to investigate the condylar morphological structure and position characteristics and differences of patients with and without TMD,so as to provide some reference for clinical prevention,diagnosis and treatment.Method:358 subjects selected respectively for TMD and TMJ healthy patients were chosen from the CBCT database of the Affiliated Stomatological Hospital of Kunming Medical University and then divided into two groups(TMD group 202 cases and healthy group 156 cases).With the cone beam volume CT scanning and imaging examining system,Mimics 10.01 software was used for temporomandibular joint three-dimensional reconstruction and measurement.Condylar measuring project include morphological parameters:condylar forms,condylar volume,condylar surface,anteroposterior diameter,mediolateral diameter;and position parameters:superior joint space,anterior joint space,posterior joint space,inner joint space,medial joint space,outer joint space.Data were analyzed by SPSS 18.0,and the relationship between the various factors and TMD were analyzed by logistic regression analysis.Results:1.The characteristics of condyle in healthy patients:(1)No statistically significant differences were observed between the right and left sides in the healthy group(P>0.05).(2)The common condylar form was rounded in TMJ healthy patients(59.17%)and there were no differences between the right and left sides.(3)The condylar volume,condylar surface,anteroposterior diameter and medialateral diameter were significantly higher in males than in females(P<0.05).2.The characteristics of condyle in TMD patients and the differences between TMD and healthy patients:(1)There were significant differences between the symptomatic and asymptomatic sides in unilateral TMD in posterior joint space(P<0.05).(2)No statistically significant differences were observed between the right and left sides in bilateral TMD(P>0.05).(3)The common condylar form was rounded in TMD patients(61.67%)and there were no differences between the right and left sides.(4)The analysis of condylar volume,mediolateral diameter,inner joint space and posterior joint space in two groups revealed a significant differences(P<0.05).3.The Correlation of TMD and condylar parameters:(1)The number of female TMD patients was more than male patients(OR was 0.587).(2)The adults was older,the risk of his getting TMD was lower(OR was 1.359).(3)If the condylar volume of somebody was small,the risk of his getting TMD should be high(OR was 0.985).(4)The posterior joint space was smaller,the risk of his getting TMD was lower(OR was 0.153).Conclusions:1.No significant differences were observed between the right and left sides in condylar measuring projects of healthy patients and the common condylar form is rounded.But the condylar volume,condylar surface,anteroposterior diameter and medialateral diameter were significantly higher in males than in females.2.There were significant differences on condylar position between the symptomatic and asymptomatic sides in unilateral TMD,but no significant differences were observed between the right and left sides in condylar measuring projects of bilateral TMD and the common condylar form is rounded.3.The condylar volume,mediolateral diameter,inner joint space and posterior joint space of bilateral TMD patients were smaller than TMJ healthy patients.4.The female,the young and the one who have small condylar volume and posterior joint space were liable to TMD.
