ObjectiveThe literature systematic evaluation method was used to clarify the distribution of clinical symptoms of coronary heart disease with intermingled phlegm and blood stasis syndrome.Through the tongue diagnosis instrument,face diagnosis instrument,pulse apparatus and questionnaire,measure the typical tongue demonstration,pulse condition,facial complexion of coronary heart disease with the phlegm and blood stasis syndrome.Explore the possibility of quantification on coronary heart disease with intermingled phlegm and blood stasis syndrome.Methods1 Systematic review of literatureDetermine search term,search Chinese Journal Full-text Database(CNKI)database,Chinese Biomedical Literature Database(CBM),and manual search for cross-checking related documents.Document inclusion and exclusion criteria were established and document system evaluation tools were established.The database was entered and statistically analyzed using the Epidata software package and the SPSS statistical analysis software package.2 Clinical study of typical tongue surface pulseTypical tongue demonstration,face colors,facial complexion,and pulse condition of clinically diagnosed patients with coronary heart disease with intermingled phlegm and blood stasis syndrome were objectively measured with tongue diagnosis instrument,face diagnosis instrument,pulse apparatus and questionnaires.The data collected were analyzed by repeated measures analysis of variance and nonlinear logistic regression.The questionnaire measured a total of 27 items,including the number of episodes of chest pain,duration of chest pain,chest pain level,chest tightness,heavy limbs,fullness,burnout fatigue,Insomnia and dreams and more.The tongue diagnosis instrument measures the content of tongue color,tongue coating color,tongue coating area,tooth marks,freckle,cracks,and stings.The contents of the face diagnosis instrument are:forehead color,nose color,cheek color,ankle color,lip color,length of the human body,and middle finger length of the same body.Pulse apparatus measurements include:pulse pressure,time index;amplitude index,angle,ratio index,heart rate.Results1 Systematic review of literature1.1 Literature search results:Retrieving CNKI and CBM databases,searching a total of1,540 articles;consulting the Guangzhou University of Traditional Chinese Medicine Library to collect monographs on traditional Chinese medicine internal medicine,integrated Chinese and Western medicine,and traditional Chinese medicine diagnostics such as coronary heart disease,angina pectoris,heart disease,chest pain,etc.Check the relevant monographs of 136 books.Determine the monographs of intermingled phlegm and blood stasis syndrome and concurrent syndrome as qualified monographs.All searched documents were screened,and the research literature that was not a coronary heart disease(CHD)syndrome was excluded.Finally,there were altogether qualified research literature:89 database articles and 67 monographs.1.2 Coronary heart disease with intermingled phlegm and blood stasis syndrome evaluation system distribution(1)Classification of disease names and results: Coronary heart disease occurred 100 times,and chest blockage occurred 49 times.It was proved that coronary heart disease(chest blockage,heartache)was more suitable for disease names,and coronary heart disease was the most common name.(2)Classification and Analysis of Syndrome Names:There are 7 types of syndromes,respectively that intermingled phlegm and blood stasis syndrome,qi deficiency and stasis-phlegm syndrome,yang deficiency and stasis-phlegm syndrome,qi and yin deficiency and stasis-phlegm syndrome,phlegm heat and blood stasis resistance,stagnation of vital energy and phlegm stasis type,cold phlegm and blood stasis.In the expression of the names of coronary heart disease with intermingled phlegm and blood stasis syndrome,the“combined evidence of deafness”is representative.(3)Etiology,pathogenesis and inducement name classification and analysis results:Involved 11 types of etiology,pathogenesis,and inducement of body obesity,exertion or seizures after activity,,hypertension,hyperlipidemia and diabetes.The top three were body obesity,exertion or seizures after activity,hypertension.(4)Classification and analysis of common symptom names:There are 36 clinical symptoms of intermingled phlegm and blood stasis syndrome.Include chest tightness/chest pain,short of breath,palpitate,fatigue etc.Among these symptoms,the top three symptoms were chest tightness/chest pain,palpitate,shortness of breath.(5)Results of tongue analysis: The tongue darkness is the typical tongue,the a representative tongue image of the coronary heart disease with intermingled phlegm and blood stasis syndrome is tongue of ecchymosis.Red tongue and plump tongue are common clinical tongue of coronary heart disease with intermingled phlegm and blood stasis syndrome.(6)Results of tongue coating analysis: The common tongue coating of coronary heart disease with intermingled phlegm and blood stasis syndrome is thegreasy fur/white and greasy tongue coating,the white tongue coating and the greasy yellow coating which is also the common tongue coating of coronary heart disease with intermingled phlegm and blood stasis syndrome.(7)Pulse analysis results: The typical pulse of coronary heart disease with intermingled phlegm and blood stasis syndrome is taut pulse.Slippery pulse,deep pulse,fineness pulse,uneven pulse and irregularly pulse are also common clinical pulse.In the complex pulse,the strings and veins pulse is the common pulse of coronary heart disease with intermingled phlegm and blood stasis syndrome.(8)The distribution rules of the four diagnoses of coronary heart disease and phlegm and convulsions were clearly defined.The typical clinical symptoms were chest tightness/chest pain,palpitation,shortness of breath,fatigue,poor appetite,fullness,etc;typical tongue is dark tongue,thegreasy fur/white and greasy tongue coating;typical pulse is pulse string,pulse slip,pulse sink or pulse slippery.1.3 An exploratory study on the syndrome rule of coronary heart disease with intermingled phlegm and blood stasis syndrome.(1)Cluster analysis resultsClinical symptoms of coronary heart disease with intermingled phlegm and blood stasis syndrome can be mainly divided into 8 categories,class1 is chest tightness/chest pain,class2 are distention and fullness,anorexia,nausea,class3 are dark complexion,lipstick purple,mouth stick,limb weight,class4 are fatigue,palpitate,shortness of breath,class5 phlegm,class6 are spontaneous sweating,cold extremities,class7 are upset/irritated,coprostasis,dry mouth,insomnia,class 8 are dizziness.(2)Factor analysis resultsClinical symptoms of coronary heart disease syndrome can be mainly 8 commonfactors,F1 fatigue,shortness of breath,palpitate,F2 upset/irritated,insomnia,coprostasis,F3 limb weight,lipstick purple,dry mouth,mouth stick,F4 distention and fullness,anorexia,nausea,F5 spontaneous sweating,cold extremities,F6 dark complexion,dizziness,F7 chest tightness/chest pain,F8 phlegm.(3)The most important syndromes of intermingled phlegm and blood stasis syndrome is chest tightness/chest pain.Six groups of symptoms that accompany the group are fatigue,shortness of breath,palpitate;distention and fullness,anorexia,nausea;lipstick purple,mouth stick,limb weight;upset/irritated,insomnia,coprostasis;upset/irritated,insomnia,coprostasis,dry mouth;spontaneous sweating,cold extremities;dark complexion,dizziness;phlegm is also typical symptoms of coronary heart disease.The above eight groups of symptoms were the distribution of typical syndromes of coronary heart disease of intermingled phlegm and blood stasis syndrome.2 Clinical study of typical tongue surface pulseIn this study,a total of 163 eligible subjects were included,including 83 patients with phlegm and blood stasis syndrome group,58 patients with non-phlegm and blood stasis syndrome group and 22 healthy patients in the health group.Patients who completed 3repeated measurements were included in the repeated measurement study.There were 24 cases in the phlegm and blood stasis syndrome group,15 cases in the non-phlegm and blood stasis syndrome group and health group only measured one time.2.1 questionnaire resultsQuestionnaire measurement entry:chest pain duration,degree of dark lip,heaviness of limbs insomnia and dreaminess on the coronary heart disease with phlegm and blood stasis syndrome group compared with the data of coronary heart disease with non-phlegm and blood stasis syndrome group,showed significant difference(P<0.05).These clinical manifestations suggest that these clinical manifestations can be used as a discriminatory reference index for coronary heart disease with intermingled phlegm and blood stasis syndrome.Comparison of typical symptoms of the questionnaire,patients with intermingled phlegm and blood stasis syndrome treated,comparison between day 5 and day 10,showed significant difference(P<0.05).It was suggested that after 10 days of treatment,this clinical manifestation was significantly improved and effective.2.2 Tongue diagnosis instrument measurement resultsRepeated measurement method was used to comparing between groups of patients with phlegm and blood stasis syndrome group,healthy group and non-phlegm and blood stasis syndrome group.The result of comparison among groups is known that the tonguecoating G value,the tongue coating area,the number and area of teeth prints,the number and length of tongue fissuring,the area and number of tongue ecchymosis showed significant difference(P<0.05).These indicators can be used as a reference for the diagnosis of coronary heart disease and sputum syndrome.The number of teeth prints,the number and length of tongue fissuring,the number and area of tongue ecchymosis compared with non-phlegm and blood stasis syndrome group showed significant difference(P<0.05).These indicators can be used as a reference indicator for differential diagnosis of coronary heart disease with phlegm and blood stasis syndrome.In the typical tongue index,repeated measurement of the tongue coating R value of coronary heart disease with phlegm and blood stasis syndrome patients,comparison between day 0 and day 10 showed significant difference(P<0.05).After treatment,the color of the tongue coating R value of the patient becomes lighter2.3 Face diagnosis instrument resultsRepeated measurement method was used to comparing between groups of patients with phlegm and blood stasis syndrome group,healthy group and non-phlegm and blood stasis syndrome group.The frontal region R value and nose R,G,B value,right cheek G,B value,lips R value,philtrum length and autofinger measurement of phlegm and blood stasis syndrome group compared with healthy group showed significant difference(P<0.05).These indicators can be used as a reference for the diagnosis of coronary heart disease and sputum syndrome.Nose G,B value,philtrum length and autofinger measurement of phlegm and blood stasis syndrome group compared with non-phlegm and blood stasis syndrome group showed significant difference(P<0.05).These indicators can be used as a reference indicator for differential diagnosis of coronary heart disease with phlegm and blood stasis syndrome.In the typical complexion index,repeated measurement of the left cheek G,B value and right cheek R,G,B value,of coronary heart disease with phlegm and blood stasis syndrome patients,comparison between day 0 and day10 showed significant difference(P<0.05).After treatment,the color of the cheek of the patient becomes lighter.After treatment the philtrum length become length,Tips for effective treatment of coronary heart disease phlegm and blood stasis syndrome.2.4 Pulse apparatus measurement resultsRepeated measurement method was used to comparing between groups of patients with phlegm and blood stasis syndrome group,healthy group and non-phlegm and blood stasis syndrome group.The hand pulse h1,h4,v Divided P data of phlegm and blood stasis syndrome groups compared with the healthy group,there was a statistically significant difference(p<0.05),These indicators can be used as a reference for the diagnosis ofcoronary heart disease and sputum syndrome.Repeated measurement of the right hand pulse h1 data of coronary heart disease with phlegm and blood stasis syndrome patients,suggested that the pressure of the systolic arterial wall was relieved after 10 days of treatment,and the ejection condition of the left ventricle was better..Repeated measurement of the right hand pulse v Divided P data of coronary heart disease with phlegm and blood stasis syndrome patients,suggested that peripheral resistance increased.2.5 Tongue,surface,pulse correlation analysis resultsNon-linear Logistic regression analysis was used to analyze the contribution of typical tongue,face and pulse measurements to the diagnosis of sputum syndrome.The results showed that the length of tongue fissuring,the number of tongue ecchymosis,the frontal region G,B value and the hand pulse t1,h1 data were significantly different(P<0.05).Prompted that the length of tongue fissuring,the number of tongue ecchymosis,the frontal region G,B value and the hand pulse t1,h1 can as a key indicator of quantitative diagnosis.Conclusion1.After a systematic review of the literature,distribution rules of the four diagnoses and sputum syndromes of coronary heart disease with intermingled phlegm and blood stasis syndrome were clearly defined.2.The possibility of objective measurement was explored.Measured by tongue diagnosis instrument,face diagnosis instrument,pulse apparatus,Patient performance of the length of tongue fissuring,the number of tongue ecchymosis,the frontal region B value and the hand pulse h1 are relevant to coronary heart disease with intermingled phlegm and blood stasis syndrome.It is clear that the above indicators can be used as quantitative indicators for the differentiation of coronary heart disease with intermingled phlegm and blood stasis syndrome.
